Keep the Attendance Certificate for your records, and apply for your MOPS or AVE <strong>CME</strong> points through your normal<br />

quarterly reporting to the RNZC<strong>GP</strong>. You may be asked for a copy of your Attendance Certificate by the RNZC<strong>GP</strong> or New<br />

Zealand Medical Council, so file in an appropriate place.<br />

While the conference is approved for up to a maximum of 24 hours <strong>GP</strong>s attending can only claim for the conference<br />

sessions they attend. Delegates will need to complete this self-recording form. This form can be kept as the Certificate<br />

of Attendance, so practitioners participating in <strong>GP</strong>EP 2 or MOPs have the necessary documentation for submission to the<br />

RNZC<strong>GP</strong> when this is requested.<br />
