Type 2 diabetes needs ongoing treatment modifications due to progressive beta cell failure. There<br />

is no doubt that optimal glycaemic control reduces or prevents the development of microvascular<br />

complications. There are various insulin preparations and regimes available but therapy should be<br />

individualised according to patient’s wishes and needs.<br />

Insulin initiation in type 2 diabetes should be considered in a timely fashion and where possible it should<br />

be commenced in primary care due to the increased prevalence of type 2 diabetes in our community.<br />

After initiation of insulin, on going titration of insulin doses to achieve desired glycaemic goals are also<br />

essential.<br />
