Provider Termination from <strong>Health</strong> <strong>Plan</strong><br />

Providers must provide written notice of termination at least 90 days in advance to <strong>Parkland</strong> CHIP Perinate, of<br />

any provider terminations. This information can be sent to:<br />

Laboratory Tests<br />

<strong>Parkland</strong> CHIP Perinate providers must refer laboratory tests to in-network facilities and contractors.<br />

Exceptions must be approved by PCHP Patient Management. Please refer to the <strong>Parkland</strong> CHIP Perinate<br />

provider directory or contact provider relations at 1-888-814-2352 for information or assistance.<br />

Coordination of Care<br />

Coordination of care is vital to assuring members receive appropriate and timely care as well as communication<br />

between providers for members who have moved out of the service area and allows for transferred care to a new<br />

HMO and provider. Compliance with this coordination is reviewed closely during site visits for credentialing<br />

and recredentialing, as well as during quality improvement and utilization management reviews.<br />

Pregnant members past the 24 th week of pregnancy will be allowed to remain under the care of their current<br />

Obstetrician/Gynecologist or pick an Obstetrician/Gynecologist within the network if she chooses to do so, if<br />

the provider to whom she wants to change agrees to accept her.<br />

Compliance with PCHP Policy and Procedures<br />

Providers will meet the program rules with all policies and procedures implemented by <strong>Parkland</strong> CHIP Perinate<br />

Utilization Management and Quality Improvement Programs.<br />
