Parkland Community Health Plan Member Identification Card
Members will get a Parkland Community Health Plan Identification (ID) card when they are enrolled in PCHP.
An example of each ID card is shown below.
Identification Cards
The front of the Parkland ID card shows important information about the member, as well as the name and
phone number of the members Primary Care Provider. It also has co-payment information, if applicable. The
back of the card has additional information, as well as important phone numbers to call if help is needed. Check
the monthly eligibility list to verify eligibility. Primary Care Providers can also verify eligibility at the time of the visit
by calling Member Services 1-888-814-2352.
