seeking prior authorization.<br />

• Payment of claims for STD/HIV services provided to members by out-of-network providers will be<br />

contingent on the transfer of treatment information by the provider to <strong>Parkland</strong> KIDSfirst and <strong>Parkland</strong><br />

CHIP Perinate Newborn.<br />

• Treatment information related to STD/HIV services provided to members by out-of-network providers will<br />

be transferred to the member’s Primary Care Provider by the <strong>Parkland</strong> KIDSfirst and <strong>Parkland</strong> CHIP<br />

Perinate Newborn Medical Director.<br />

PCHP Staff<br />

• The PCHP Staff Orientation and Employee Handbook will contain statements regarding the responsibility<br />

of the PCHP staff to keep member medical records and information secure and confidential. These<br />

statements will include information on the penalties imposed for breaching member confidentiality.<br />

• Member records and member information in PCHP possession, including information related to STD/HIV<br />

treatment or services received by the member, will be maintained under supervision during business hours<br />

and secured in locked file cabinets or a locked room during hours when the facility is closed for business.<br />

• Member medical records will be available only to PCHP staff in working directly with clinical matters,<br />

claims processing, or other payment related data included in the medical record.<br />

• PCHP must obtain the member’s written consent to release information to individuals or entities outside of<br />

PCHP when disclosure of records is requested.<br />

• Requests for release of member information related to STD/HIV treatment or services received by the<br />

member from outside <strong>Parkland</strong> KIDSfirst, <strong>Parkland</strong> CHIP Perinate or <strong>Parkland</strong> CHIP Perinate Newborn<br />

must be directed to the Medical Director.<br />

• Requests for release of information will be recorded by the Medical Director’s office.<br />

• If a properly executed written consent form does not accompany the request, the request will be denied in<br />

writing. The written denial will contain the reason for the denial and instructions on how to request the<br />

information properly. A copy of the denial will be returned to the member.<br />

• If the request for member information is accompanied by a properly executed request for release of<br />

information, the Medical Director will send the information if it is available in the facility. If not, the<br />

Medical Director will refer the request to the member’s Primary Care Provider for action. This information<br />

will be addressed to the requesting official via First Class U.S. mail in a sealed envelope marked<br />

CONFIDENTIAL. A copy of the letter accompanying the released information will be sent to the member<br />

and the member’s Primary Care Provider.<br />

• PCHP members can review or obtain their own health care information by sending a written signed request<br />

to the PCHP Medical Director. The request must include the member’s name, date of birth, member ID<br />

number. A copy of the request will be forwarded to the member’s Primary Care Provider.<br />

• PCHP will respond in writing, within 5 business days of the date of receipt of the request, to written request<br />

by the member who asks to review their health care information.<br />
