By registering for an (ISC)² certification examination, I hereby affirm that I understand, acknowledge and agree to abide to the following
policies attached to this application (pages 1-25).
受 験 登 録 手 続 きを 行 うことにより、 申 込 者 は、ここに、 本 申 込 書 (1~25 頁 )に 添 付 されている 以 下 の 方 針 を 理 解 し、 了 承 の 上 で、これに 同 意 するこ
とを 確 約 します。
I have read the (ISC)² Code of Ethics and agree to abide by its provisions. I have also read the applicant requirements as listed in this registration form
and in the associated Candidate Information Bulletin published on the (ISC)² Website and agree that I meet each and every requirement set forth and
have completely, honestly and accurately completed this registration form to the best of my knowledge. (ISC)² may, at its sole discretion, make inquiry of
individuals and organizations directly or indirectly referenced in any part of this application to verify the accuracy and completeness of this information I
have provided. I further agree to cooperate in any such investigation by (ISC)² regarding the information I have provided, including my criminal history. I
understand that providing any information that is fraudulent, or failing to completely or accurately disclose facts known to me, or my failure to cooperate in
any inquiry by (ISC)² into the information I have provided, will result in the refusal of (ISC)² to issue the credential to me or revocation of my credential if
already awarded, and me being forever barred from ever attaining an (ISC)² credential.

に、 正 当 に、 正 確 に 記 載 されていることに 同 意 します。 (ISC) 2 は、 独 自 の 裁 量 により、 申 込 者 が 申 告 した 情 報 の 正 確 さと 完 全 性 を 検 証 するために、 本
申 込 書 上 で 直 接 、もしくは 間 接 的 に 記 載 されている 個 人 および 組 織 に 対 して 問 い 合 わせを 行 なうことができます。また 申 込 者 は、 犯 罪 歴 を 含 めて、 申
込 者 の 申 告 した 情 報 に 関 する(ISC) 2 の 調 査 に 協 力 することに 同 意 します。 申 込 者 は、 虚 偽 の 情 報 を 申 告 した 場 合 、もしくは 申 込 者 の 知 る 事 実 を 完 全 ま
たは 正 確 に 開 示 しなかった 場 合 、 申 込 者 の 申 告 した 情 報 に 関 する(ISC) 2 の 問 い 合 わせに 協 力 しなかった 場 合 、(ISC) 2 が 申 込 者 への 資 格 付 与 を 拒 否 するこ
と、または 既 に 付 与 されている 場 合 には、(ISC) 2 認 定 資 格 を 剥 奪 することになり、 申 込 者 は 永 久 に 本 資 格 を 取 得 できなくなることを 理 解 しています。

Any action arising out of the application, the examination, or the certification must be brought in the Circuit Court of Suffolk County, Massachusetts, USA
and shall be governed by the laws of the State of Massachusetts.
本 申 込 書 、 試 験 、もしくは 認 定 を 原 因 とするいかなる 訴 訟 も、アメリカ 合 衆 国 マサチューセッツ 州 サフォーク 郡 巡 回 裁 判 所 に 提 起 しなければならず、

(ISC)² will not accept third party payment or applications for any (ISC)² examination applicant unless the third party is a government agency, the
applicant's employer, or is otherwise authorized by (ISC)² prior to the application being submitted.
