EXAMINATION REGISTRATION FORM - ISC

(ISC)² reserves the right to cancel any exam 15 days in advance if attendance is insufficient. In this case, (ISC)² liability shall
be limited to a full refund of fees paid. If the candidate chooses to reschedule exam or seminar instead of receiving a refund,
the candidate will have 365 days to reschedule. The burden is on the candidate to track when the one year period expires,
as (ISC)² will not notify the candidate.
(ISC)²は 受 験 者 数 が 尐 ない 場 合 に、15 暦 日 前 に 試 験 を 中 止 する 権 利 を 有 し、その 場 合 、(ISC)²の 責 任 は、 受 領 済 み 受 験 費 用 の 全 額 払 い 戻 し
に 限 定 されるものとします。もし、 申 込 者 が 払 い 戻 しの 代 わりに、 試 験 あるいはセミナーの 日 程 変 更 を 希 望 する 場 合 、 日 程 変 更 は 当 初 受 験
予 定 の 試 験 日 から365 暦 日 以 内 に 受 験 するものとします。この 期 日 管 理 は、 申 込 者 自 身 で 行 って 下 さい。1 年 後 の 期 限 になっても(ISC)²から
連 絡 はありません。
If Candidate cancels or reschedules an exam:
申 込 者 が 申 し 込 みを 取 り 消 し(キャンセル)あるいは 日 程 変 更 する 場 合
All cancellation or rescheduling requests received in writing with 22 days notice or more will incur a JPY13,650 cancellation fee
or rescheduling fee (Refund = Amount Paid, Less JPY13,650). Written cancellation or rescheduling requests received five
calendar days or more prior to the exam will be given credit toward attendance at a subsequent program only (no refund) and
will incur an additional JPY13,650 rescheduling fee.
22 暦 日 以 上 の 書 面 による 事 前 通 知 で 申 込 取 消 、もしくは、 受 験 日 程 変 更 の 要 請 があった 場 合 、 申 込 取 消 もしくは 受 験 日 程 変 更 の 手 数 料 とし
て13,650 円 ( 税 込 )を 申 し 受 けます( 払 戻 額 = 支 払 額 から 手 数 料 を 控 除 した 金 額 )。 受 験 日 の5 暦 日 かそれより 前 に 書 面 で 申 込 取 消 もしく
は 受 験 日 変 更 の 要 請 があった 場 合 、 以 降 のプログラムへの 参 加 のみを 認 め( 払 戻 しなし)、 受 験 日 程 変 更 について 追 加 手 数 料 13,650 円 ( 税
込 )を 申 し 受 けます。
Cancellations received with less than five calendar days notice and "no-shows" will not be given a refund, nor credit toward a
later program (The only exception is a medical emergency which prevents the applicant from sitting. Appropriate
documentation from a licensed medical professional must be submitted within 30 days after the "no-shows" examination date).
Rescheduling or canceling both a review seminar and an exam will incur two JPY13,650 fees for a total of JPY27,300.
5 暦 日 未 満 の 事 前 通 知 での 申 込 取 消 要 請 、もしくは『 無 断 欠 席 』の 場 合 、その 後 のプログラムについて 払 戻 しも 行 わず、 受 講 も 認 めません。
( 唯 一 の 例 外 は、 医 学 的 な 緊 急 事 態 により 受 験 することができなかった 場 合 です。その 場 合 には、 欠 席 をした 試 験 日 から30 暦 日 以 内 に、 医
師 による 適 切 な 証 明 書 類 の 提 出 が 必 須 です。)(ISC)² 公 式 セミナーおよび 試 験 の 両 方 の 日 程 変 更 、もしくは 申 込 取 消 の 場 合 、13,650 円 ずつ、
合 計 27,300 円 の 手 数 料 を 申 し 受 けます。
Note: (ISC)² will not accept third party payments or applications for any (ISC)² examination applicant unless the third party is a
government agency, the applicant's employer, or is otherwise authorized by (ISC)² prior to the application being submitted.

1.6 Re-Testing 再 受 験
(ISC)² uses the term "retake" to identify a candidate who has previously sat for an examination and now wishes to make
another attempt. You must complete, execute and re-submit the examination registration form to retake an examination. This
form provides (ISC)² with your contact information and requires that you demonstrate the required experience, and select your
test site and date. You must also submit the appropriate fee. (ISC)² does not impose any type of "waiting period" between
retake attempts.
(ISC)²は、 以 前 に 試 験 を 受 験 したことがあり、 再 度 受 験 を 希 望 する 受 験 者 を「 再 受 験 」と 定 義 します。 再 受 験 をするには、 試 験 登 録 申 込 書
に 記 入 ・ 署 名 し、 再 度 提 出 しなければなりません。 申 込 者 の 連 絡 先 を 試 験 申 込 書 に 記 入 し、 要 件 を 満 たす 業 務 経 験 があることを 示 し、 試 験
会 場 ・ 試 験 日 を 選 択 して 下 さい。 受 験 費 用 も 再 度 支 払 う 必 要 があります。(ISC)²では、 受 験 日 と 受 験 日 の 間 の「 待 機 期 間 」については 特 に
規 定 を 設 けていません。
1.7 Recertification by Examination 試 験 による 認 定 更 新
It is the policy of (ISC)² to prohibit the retaking of any (ISC)² examination by a certificate holder more than 90 days prior to the
expiration of the holder’s certificate (i.e., to audit the exam). Furthermore, a certificate holder may not retake an examination if
(1) he/she has been decertified by (ISC)² and prohibited from being recertified; (2) he/she possesses the requisite number of
Continuing Professional Education (CPE) credits for recertification; (3) he/she owes any Annual Maintenance Fees (AMFs); or
(4) he/she currently serves, or intends to serve within the next 90 days, as an instructor or advisor preparing others for the
examination, whether for (ISC)² or any other organization. Violation of any provision of this policy shall be submitted to the
(ISC)² Professional Practices Committee for remedial action, including possible decertification.
(ISC)²の 方 針 で、CISSP 認 定 保 持 者 の 資 格 失 効 期 限 より90 日 以 上 前 に、CISSP 認 定 保 持 者 が(ISC)² 認 定 試 験 を 再 受 験 することは 禁 じられて
います。また、(1)CISSP 認 定 保 持 者 が(ISC)²から 既 に 取 り 消 しを 受 けてその 更 新 を 禁 じられている 場 合 、(2)CISSP 認 定 保 持 者 が 証 明
書 の 資 格 更 新 の 要 件 となる 継 続 教 育 単 位 (CPEクレジット)を 保 有 している 場 合 、(3)CISSP 認 定 保 持 者 に 未 払 いの 年 会 費 が 有 る 場 合 、
(4)(ISC)²であるかその 他 の 組 織 であるかにかかわらず、(ISC)² 公 式 セミナーの 講 師 もしくは 受 験 希 望 者 へのアドバイザーを 務 めている、
または 今 後 90 日 以 内 に 務 める 予 定 がある 場 合 、かかるCISSP 認 定 保 持 者 は 再 受 験 することができません。かかる 方 針 の 条 項 のいずれかに 関
する 違 反 は、CISSP 資 格 の 剥 奪 の 可 能 性 を 含 めて、 是 正 措 置 のために「(ISC)² 専 門 家 実 務 委 員 会 (Professional Practices Committee)」に
付 託 するものとします。
