members are very strict and do not allow me for this”<br />

• Fear of injections<br />

“I do not get TT vaccine in any of my pregnancies<br />

because I am scared of injection. The needle causes<br />

pain” (FGD-Darvesh, Haripur)<br />

• Lack of time<br />

“I do not go for TT injections because of the household<br />

chores I cannot get time and the family members also<br />

do not allow” (FGD-Umer Zai)<br />

• Misconceptions about side effects (NWFP)<br />

“I did not get TT vaccine during the first two<br />

pregnancies. This time I went for immunization and<br />

now my baby has hundreds of diseases. He has<br />

asthma; his liver doesn’t form blood and many others.<br />

The first two children are perfectly alright and they do<br />

not have any disease.”(FGD-Wasti, Haripur)<br />

“If the mother goes for TT injections during pregnancy<br />

then all the diseases from the mother’s womb are<br />

transferred to the baby” (IDI, Mo-Ali Khan, Haripur)<br />

• Culturally not practiced (NWFP)<br />

“In our village there is no tradition of taking T.T<br />

injections, or taking any blood test and taking any<br />

nutritional supplements” (FGD-Landiwah, Lakki<br />

Marwat)<br />

3.2 Birth Attendants and Their Practices<br />

Most of the deliveries take place at home and<br />

are conducted by daiyan (Punjab and Sindh and Balochistan<br />

urban), by a family member (mostly in NWFP districts<br />

and Balochistan rural) or by the women themselves (few<br />

reported this in ….. ). Only in few urban areas in Sukkur,<br />

DG Khan, Pishin it was mentioned that delivery takes place<br />

in a health facility.<br />

“Everybody delivers with daiyan here and if the need<br />

be she takes the mother to a hospital” (FGD-Janjeel,<br />

Jhelum).<br />

“If someone can afford then dai is called for delivery.<br />

Otherwise deliveries are conducted at home by<br />

mother-in-law or sister-in-law.” (FGD- Faizabad)<br />

“Deliveries are conducted at home because it costs<br />

very less money. Dai does not charge much.” (IDI,<br />

Husband- Faizabad)<br />

“I delivered all three kids with my mother-in-law” (FGD-<br />

Sarai Saleh, Haripur).<br />

“There is an elder relative who conducts deliveries,<br />

there is no dai in the village and all the babies are<br />

delivered with her” (FGD-Tulanda, Charsadda).<br />

“We help each other in delivering the babies;<br />

husbands’ sisters and wives of brothers” (FGD-Tulanda,<br />

Charsadda).<br />

“Most of the time a woman in the family like motherin-law<br />

and sisters-in-law, whoever is present at home,<br />

assist in delivering the baby but in some households<br />

dai is also called” (FDG-Landiwah, Lakki Marwat).<br />

“My baby was born at home. We did not call any<br />

doctor or dai nor did the women in the house help me<br />

with delivery. I did all the work whole day and in the<br />

evening I went in the room for something, suddenly<br />

pain started, and the baby came out. I held her in my<br />

hand and when the placenta came out I detached it.<br />

After that I called my sister-in-law and handed her the<br />

baby. She took care of the baby and cut her cord” (IDI,<br />

Mo-Landiwah, Lakki Marwat)<br />

“Deliveries at hospital are a matter of shame. At home<br />

everything happens in purdah but at hospital it is a<br />

matter of shame.” (IDI, Husband- Ghazgi, Khuzdar)<br />

“Many people now do not have deliveries at homes,<br />

they deliver at hospital” (FGD- Shakir Town, DG Khan)<br />

Those who go to the health facilities for deliveries do<br />

not trust a dai and question her ability to take care of the<br />

newborn if s/he has a problem at the time of birth. They<br />

also believe that a dai does not conduct the delivery with<br />

cleanliness.<br />

“If some problem occurs then what can a dai do In<br />

hospitals there are doctors and nurses. Dai does not<br />

know but the doctors can understand the problems”<br />
