Activities FACHC undertakes <strong>to</strong> accomplish community development goals include working with<br />

<strong>FQHCs</strong> <strong>and</strong> communities <strong>to</strong> help them:<br />

• Underst<strong>and</strong> FQHC funding, “Look Alike” <strong>and</strong> related opportunities<br />

• Underst<strong>and</strong> federal expectations <strong>and</strong> requirements of FQHC programs<br />

• Locate data <strong>to</strong> help determine or demonstrate levels of need<br />

• Achieve the highest possible score on the Need for Assistance (NFA) worksheet<br />

• Establish the necessary foundations for becoming an FQHC including building community<br />

support <strong>and</strong> establishing a community led Board of Direc<strong>to</strong>rs.<br />

• Develop or strengthen partnerships between <strong>FQHCs</strong> <strong>and</strong> other organizations<br />

• Link communities <strong>and</strong> <strong>FQHCs</strong> with <strong>330</strong> grant writers <strong>and</strong> other consultants<br />

FACHC accomplishes these goals through:<br />

• Statewide, regional <strong>and</strong> local trainings, meetings <strong>and</strong> events<br />

• Conference calls<br />

• On-going email <strong>and</strong> telephone technical assistance<br />

• Written materials<br />

• FACHC’s web site<br />

Since 2004, FACHC has provided community development assistance <strong>to</strong> the following counties:<br />

There is a great deal of unmet need for healthcare services in Florida, particularly for low income<br />

people <strong>and</strong> the uninsured. As the state’s primary care association FACHC works with <strong>FQHCs</strong> <strong>and</strong><br />

other safety net providers <strong>to</strong> increase access <strong>to</strong> quality healthcare services. FACHC’s strategic<br />

planning, needs assessment <strong>and</strong> community development activities help set priorities <strong>and</strong> guide our<br />

efforts <strong>to</strong> increase access <strong>to</strong> care. FACHC, in partnership with the Florida Department of Health, is<br />

actively working with communities <strong>and</strong> <strong>FQHCs</strong> throughout the state on the expansion of affordable<br />

quality healthcare services. Significant progress has been made but there is still much left <strong>to</strong> do.<br />
