Health Disparities Collaboratives (HDC) - A federal initiative aimed at eliminating health<br />

disparities for underserved Americans who suffer disproportionally from chronic diseases including<br />

diabetes, hypertension, asthma, cancer <strong>and</strong> depression.<br />

Health Professional Shortage Area (HPSA) - A federal designation documenting a shortage of<br />

health care providers (a minimum population <strong>to</strong> provider ratio of at least 3000:1) <strong>and</strong> the existence<br />

of barriers <strong>to</strong> accessing care.<br />

Healthy People 2010 (HP 2010)- A Federal initiative focused on a comprehensive set of health<br />

objectives that serve as a roadmap for increasing the quality <strong>and</strong> years of healthy life for all<br />

Americans <strong>and</strong> eliminating disparities in health status between different groups of people.<br />

Homeless - an individual who lacks housing including an individual whose primary residence<br />

during the night is a supervised public or private facility that provides temporary living<br />

accommodations <strong>and</strong> an individual who is a resident in transitional housing.<br />

HRSA - Health Resources Services Administration, the federal agency that funds the <strong>330</strong> program.<br />

Integrated Service Delivery Network (ISDN) - A network of <strong>FQHCs</strong> that shares business services<br />

creating economies of scale resulting in higher levels of efficiency.<br />

J-1 Visa Waiver Program - Allows foreign medical graduates <strong>to</strong> obtain a waiver of the two-year<br />

home residence requirement provided they agree <strong>to</strong> work for three years in a designated medically<br />

underserved or health professional shortage area (MUA/MUP or HPSA)<br />

JCAHO - Joint Commission on Accreditation of Health Care Organizations provides health care<br />

accreditation that support performance improvement in health care organizations<br />

Language Access - Ability of non speaking people <strong>to</strong> access healthcare services in a language they<br />

underst<strong>and</strong>.<br />

Medically Underserved Area (MUA) / Medically Underserved Population (MUP) - A federal<br />

designation indicating that a geographic area or population is medically underserved based on (1)<br />

percent of the population below 100 percent of the Federal Poverty Level, (2) percent of the<br />

population over age 65, (3) infant mortality rate (5 year average) <strong>and</strong> (4) population-<strong>to</strong>-physician<br />

ratio. All <strong>FQHCs</strong> are required <strong>to</strong> serve an MUA or MUP.<br />

Migrant <strong>and</strong> Seasonal Farmworker (MSFW) - A person or their dependents who earns at least<br />

51% of their income from agricultural work.<br />

National Health Service Corps (NHSC) - A federal program that matches physicians <strong>and</strong> other<br />

clinicians with communities designated at Health Professional Shortage Areas (HPSA)<br />

Need for Assistance Worksheet (NFA) - Part of the <strong>330</strong> application process which assists<br />

communities in identifying the unique health issues <strong>and</strong> disparities as well as other fac<strong>to</strong>rs<br />

impacting access <strong>to</strong> care for their communities.<br />
