What Criteria Are Used <strong>to</strong> Review the NAP Application<br />

NAP application guidelines include a set of eight criteria reviewers use <strong>to</strong> score each proposal <strong>and</strong><br />

evaluate its strengths <strong>and</strong> weaknesses. Criteria are divided in<strong>to</strong> the following categories <strong>and</strong> are<br />

assigned the corresponding maximum number of points:<br />

1. Description of The Service Area/Community And Target Population (Maximum 10<br />

Points) (3-4 Pages)<br />

2. Strategic Planning (Maximum 10 Points) (2-3 Pages)<br />

3. Service Delivery Strategy And Model (Maximum 20 Points) (4-5 Pages)<br />

4. Health Care Services (Maximum 15 Points) (5-6 Pages)<br />

5. Organizational Capabilities And Expertise (Maximum 15 Points) (3-4 Pages)<br />

6. Budget (Maximum 10 Points) (2-3 Pages)<br />

7. Governance (Maximum 10 Points) (2-3 Pages)<br />

8. Readiness (Maximum 10 Points) (1-2 Pages)<br />

What Criteria Are Used To Score And Evaluate The “Description Of The Service Area,<br />

Community And Target Population” (Criteria #1) (Maximum 10 Points)<br />

1. Applicant describes the service area(s)/community(ies) <strong>to</strong> be served by the NAP(s):<br />

(a) Identify the counties, census tracts, minor civil divisions, schools/school<br />

districts, etc., (as appropriate) in the service area.<br />

(b) Discuss any geographic fac<strong>to</strong>rs <strong>and</strong>/or culturally specific characteristics that<br />

impact access <strong>to</strong> <strong>and</strong> the delivery of health care services.<br />

(c) Describe any relevant geographic barriers <strong>to</strong> care <strong>and</strong> other fac<strong>to</strong>rs impacting<br />

access <strong>to</strong> care.<br />

(d) Describe any significant changes over the past year in the service area or<br />

population being served (i.e., influx of refugee population, or closing of local<br />

hospital, etc.)<br />

2. Applicant describes the target population(s) (e.g., general community members,<br />

migrant/seasonal agricultural workers, residents of public housing, homeless persons,<br />

low-income school children, etc.) within the service area/community:<br />

(a)<br />

(b)<br />

(c)<br />

(e)<br />

(f)<br />

(g)<br />

Identify the unserved <strong>and</strong> underserved populations in the community.<br />

Describe the unique demographic characteristics of the target population (e.g.,<br />

age, gender, insurance status, unemployment, poverty level, ethnicity/culture,<br />

education, etc.).<br />

Define any special health care needs of the target population(s).<br />

Identify any other populations that are in need of access <strong>to</strong> primary health care<br />

(e.g., migrant/seasonal farmworkers, homeless populations, residents of public<br />

housing, low-income school children/adolescents <strong>and</strong> their families).<br />

Describe relevant access <strong>to</strong> care <strong>and</strong> health status indica<strong>to</strong>rs of the target<br />

population/community. These should be indicated in the Need for Assistance<br />

Worksheet <strong>and</strong> addressed in the Health Care Plan.<br />

Describe the unique characteristics <strong>and</strong> other fac<strong>to</strong>rs of the target population.<br />
