• Health centers that propose <strong>to</strong> exp<strong>and</strong> medical capacity by initiating or exp<strong>and</strong>ing only a<br />

MHC, HCH, <strong>and</strong>/or PHPC program, <strong>and</strong> do not also include a request for new or additional<br />

CHC funding, must demonstrate a minimum increase in users of 10 percent of the <strong>to</strong>tal<br />

number of users for the last calendar year UDS, or 1000 new users – whichever is less.<br />

What Are HRSA’s Expectations of EMC Applicants<br />

Applicants are expected <strong>to</strong>:<br />

• Present a sound <strong>and</strong> complete plan for the successful expansion of their medical capacity<br />

that demonstrates responsiveness <strong>to</strong> the identified needs of the target population within the<br />

existing service area.<br />

• Demonstrate a significant increase in penetration in<strong>to</strong> underserved communities or<br />

populations.<br />

• Demonstrate readiness <strong>to</strong> initiate the proposed project, i.e., proposed staff recruited <strong>and</strong><br />

services provided <strong>to</strong> the community within 120 days of receiving the grant award.<br />

• Present a reasonable <strong>and</strong> appropriate budget based on the activities proposed in the project<br />

plan <strong>and</strong> business plan. The proposed budget should be based on an appropriate increase in<br />

medical users.<br />

• Describe systems that ensure participation of the diverse cultures in their communities,<br />

including participation of persons with limited English-speaking ability<br />

• Demonstrate that the proposed clinical staffing pattern (e.g., number <strong>and</strong> mix of primary care<br />

physicians <strong>and</strong> other providers <strong>and</strong> clinical support staff, linguistic <strong>and</strong> cultural competency,<br />

etc.) is appropriate for the level <strong>and</strong> mix of services <strong>to</strong> be provided.<br />

• Demonstrate continued compliance with the Health Center Program Expectations (Program<br />

Information Notice 98-23).<br />

Can EMC Funds Be Used To Exp<strong>and</strong> Mental Health And Substance Abuse Services<br />

Mental Health <strong>and</strong> Substance Abuse providers can be included in the EMC only where necessary <strong>to</strong><br />

support medical providers when mental health <strong>and</strong> substance abuse problems are prevalent in the<br />

new populations being served. All other proposals <strong>to</strong> add new services should be submitted through<br />

a <strong>330</strong> Service <strong>Expansion</strong> application, which is a separate competition.<br />

Are There Any Special Considerations for Applicants Proposing <strong>to</strong> Serve Special<br />

Populations<br />

Migrant Health Center (MHC) (<strong>330</strong>g) applicants must address how the special needs of migra<strong>to</strong>ry<br />

<strong>and</strong> seasonal farmworkers (MSFW) <strong>and</strong> their families will be met <strong>and</strong> how services will be made<br />

available through mechanisms such as outreach that is integrated in<strong>to</strong> the primary health care<br />

delivery system; use of existing mobile vans or health teams that travel <strong>to</strong> migrant camps;<br />

transportation; extended clinic hours; etc. Migrant EMCs must also demonstrate how the special<br />

environmental health concerns that are associated with MSFWs will be addressed <strong>and</strong> how all<br />

applicable BPHC regulations <strong>and</strong> guidelines, including the governance requirement, will be met.<br />

Health Center for the Homeless (HCH) (<strong>330</strong>h) applicants must indicate the mechanism for<br />

delivering substance abuse services <strong>to</strong> homeless patients <strong>and</strong> describe the manner in which<br />

comprehensive outreach is <strong>to</strong> be conducted, <strong>and</strong> how transportation <strong>and</strong> other enabling services<br />
