commitment or investment (e.g., from the school board, local hospital, public health<br />

department), etc., as part of the application attachments.<br />

4. Applicant discusses the extent <strong>to</strong> which the proposed service delivery model will increase<br />

access <strong>to</strong> primary health care services for the medically underserved in the<br />

community/target population.<br />

5. Applicant discusses the extent <strong>to</strong> which the proposed service delivery model will, <strong>and</strong><br />

demonstrates a clear plan <strong>to</strong>, reduce or eliminate disparities identified in the population<br />

or community <strong>to</strong> be served.<br />

6. Applicant describes how the proposed new access point(s) is a cost-effective approach <strong>to</strong><br />

meeting the primary care needs of the target population given the health care needs of<br />

the target population <strong>and</strong> the level of health care resources currently available in the<br />

community.<br />

7. Applicant demonstrates, in cases where the site(s) are already operational, how section<br />

<strong>330</strong> funds will augment/supplement existing services, resources <strong>and</strong> providers <strong>to</strong><br />

exp<strong>and</strong> accessibility <strong>and</strong> availability of primary health care services <strong>to</strong> underserved<br />

populations.<br />

In addition <strong>to</strong> the above criteria, applicants requesting funding for a new access point(s) for the<br />

following types(s) of health center must also respond <strong>to</strong> the following criteria:<br />

FOR MHC APPLICANTS:<br />

(a) Applicant describes an outreach program that will increase access <strong>to</strong> primary <strong>and</strong><br />

preventive health care services <strong>and</strong> how the outreach program is integrated in<strong>to</strong> the<br />

primary care delivery system.<br />

FOR SBHC APPLICANTS:<br />

(a) Applicant provides documentation of access <strong>to</strong> health care during the summer.<br />

FOR PHPC APPLICANTS:<br />

(a) Applicant documents a formal agreement with the local Public Housing Authority that<br />

demonstrates the sufficient provision of comprehensive services.<br />

FOR HCH APPLICANTS:<br />

(a) Applicant describes, for each service offered, the service setting (e.g. permanent<br />

site, mobile van, shelter site, etc.)<br />
