New Access Point (NAP) - a new FQHC delivery site which may either be a “Satellite” of an<br />

existing FQHC or a “New start” which is a completely new FQHC organization.<br />

Outreach - Coordinated effort by an FQHC <strong>to</strong> “Reach out” <strong>to</strong> populations in need of healthcare<br />

services.<br />

Policy Information Notice (PIN) - An official federal guidance. HRSA PINS can be found at<br />

http://bphc.hrsa.gov/pinspals/pins.htm.<br />

Program Expectations - Federal expectations <strong>and</strong> minimum requirements for <strong>FQHCs</strong> outlined in<br />

PIN 98-23.<br />

Prospective Payment System (PPS) - A payment system established by congress which allows<br />

states <strong>to</strong> set Medicaid reimbursement rates <strong>to</strong> ensure adequate Medicaid reimbursement at CHCs.<br />

Performance Review Pro<strong>to</strong>col - A process <strong>to</strong> improve performance of HRSA funded programs by<br />

working collaboratively <strong>to</strong> measure program performance, analyze the fac<strong>to</strong>rs impacting<br />

performance, <strong>and</strong> identify effective strategies <strong>and</strong> partnerships <strong>to</strong> improve performance, with a<br />

particular focus on outcomes.<br />

Primary <strong>and</strong> Preventative Healthcare - Includes basic medical care, health screenings, prenatal<br />

care, family planning, pediatrics, immunizations, emergency medical services, vision services,<br />

diagnostic X-ray <strong>and</strong> labora<strong>to</strong>ry services.<br />

Service Area - The geographic or “catchment” area served by an FQHC.<br />

Scope of Service - Officially defines the range of services <strong>and</strong> geographic area served by an FQHC.<br />

Special Population- Populations with special barriers <strong>to</strong> healthcare access. Categories of <strong>330</strong><br />

funding for special populations include homeless, migrant <strong>and</strong> seasonal farmworkers, public<br />

housing residents <strong>and</strong> school children <strong>and</strong> their families.<br />

Target population - A high need population targeted for services by an FQHC. This may include<br />

low income people, the uninsured, the elderly, school children, the unemployed, farmworkers,<br />

homeless etc.<br />

Uniform Data System (UDS) - A core set of information reported by every FQHC allowing<br />

moni<strong>to</strong>ring <strong>and</strong> evaluation of health center performance <strong>and</strong> trends including provider productivity,<br />

patient demographics, key diagnoses, service utilization <strong>and</strong> fiscal issues.<br />
