continue <strong>to</strong> maximize access <strong>to</strong> health care for the community currently being served by the<br />

existing site.<br />

3. Applicant provides a Service Delivery Plan that is appropriate <strong>and</strong> realistic for the proposed<br />

EMC project, <strong>and</strong> responsive <strong>to</strong> the identified needs of the community, including any targeted<br />

health disparities. The plan includes adequate medical <strong>and</strong> other providers (e.g., outreach<br />

workers, case managers) <strong>and</strong> support staff associated with the expansion. The Service<br />

Delivery Plan addresses its impact on the current Heath Care Plan for the organization, <strong>and</strong><br />

any significant changes (e.g., serving a new population or age group, or exp<strong>and</strong>ing in<strong>to</strong> new<br />

services) are noted in an addendum <strong>to</strong> the existing Health Care Plan.<br />

4. Applicant provides a Business Plan that is appropriate <strong>and</strong> realistic for the proposed EMC<br />

project. Each relevant component of the Heath Center (e.g., Administration, Fiscal,<br />

Clinical, MIS, Governance, etc.) is sufficiently addressed, <strong>and</strong> the goals <strong>and</strong> activities are<br />

appropriate for the proposed project.<br />

5. Applicant proposes appropriate plans for incorporating or improving the use of outreach <strong>and</strong><br />

other enabling services <strong>to</strong> bring new users in <strong>to</strong> the health centers <strong>and</strong>/or medical services,<br />

both for traditional health center users <strong>and</strong>, where applicable, for special populations (e.g.,<br />

MSFWs, homeless individuals <strong>and</strong> families, residents of public housing).<br />

6. Applicant demonstrates that the new or exp<strong>and</strong>ed services being proposed are culturally<br />

<strong>and</strong> linguistically appropriate for the populations <strong>to</strong> be served.<br />

7. For organizations proposing an expansion <strong>to</strong> serve a population for which they do not<br />

currently receive Federal support, the applicant:<br />

(a) Demonstrates awareness of the appropriate organizational changes <strong>to</strong> be made (per<br />

statu<strong>to</strong>ry <strong>and</strong> other requirements <strong>and</strong> expectations of health centers). Areas of<br />

consideration include representation in governance, <strong>and</strong> modifications <strong>to</strong> clinical <strong>and</strong><br />

business systems, enabling services <strong>and</strong> overall cultural competency.<br />

(b) Presents a sound plan <strong>to</strong> assure compliance upon receipt of funding.<br />

(c) Assures the organization’s ability <strong>to</strong> implement the necessary changes.<br />

8. Applicant provides a feasible <strong>and</strong> credible timeline for implementation of the project. The<br />

applicant demonstrates operational readiness within 120 days of award (i.e., proposed staff<br />

recruited <strong>and</strong> services provided), including those applicants proposing <strong>to</strong> open a<br />

replacement site.<br />

What Criteria Are Used To Score And Evaluate The “Evaluative Measures” Section (Criteria<br />

#3) (Maximum 10 Points)<br />

1. Applicant demonstrates the ability <strong>to</strong> evaluate the effectiveness of the EMC project,<br />

including the quality <strong>and</strong> outcomes of the services provided (e.g., adequate management<br />

information systems, patient satisfaction). Applicant provides an evaluation plan that is<br />

appropriate for the EMC project goals <strong>and</strong> projected outcomes.<br />
