2. Applicant describes the extent <strong>to</strong> which Federal section <strong>330</strong> funds will be used <strong>to</strong> exp<strong>and</strong><br />

services <strong>and</strong> capacity <strong>to</strong> provide services. If the applicant is purchasing/taking<br />

over/assuming a clinic(s) operation as part of the proposal, the applicant must demonstrate<br />

the level of continued commitment <strong>and</strong> collaboration of the other organization <strong>to</strong> assure<br />

the viability of the clinic operation.<br />

3. Applicant demonstrates that the Federal grant funds requested are being used <strong>to</strong> leverage<br />

other sources of funding <strong>and</strong> that alternative sources of revenue (i.e., Medicaid, SCHIP,<br />

<strong>and</strong> other third party resources) are being maximized <strong>to</strong> support the operation of the new<br />

access point(s).<br />

4. Applicant demonstrates that the Business Plan goals <strong>and</strong> objectives are targeted <strong>to</strong> activities<br />

related <strong>to</strong> the new access point <strong>and</strong> demonstrate appropriate financial planning in the<br />

development of the proposal <strong>and</strong> for the long-term success of the project.<br />

In addition <strong>to</strong> the above criteria, applicants requesting funding for a new access point(s) for<br />

the following types(s) of health center must also respond <strong>to</strong> the following criteria:<br />

FOR SBHC APPLICANTS:<br />

(a) Applicant itemizes the estimated value of in-kind resources (including equipment, rent,<br />

personnel, renovations <strong>and</strong> alterations).<br />

FOR PHPC APPLICANTS:<br />

(a) Applicant demonstrates no more than 20 percent of the grant funds will be used for<br />

outreach activities.<br />

Are There Any Specific <strong>Guide</strong>lines For Developing The Budget<br />

The following is provided as a guideline in developing the Federal budget request: $150 Federal grant<br />

dollars per general community user, $200 Federal grant dollars per migrant/seasonal farmworker<br />

user, homeless user, public housing user <strong>and</strong> school-based health center user.<br />

The budget should be consistent with the health care <strong>and</strong> business plans presented in the application.<br />

Form 424A is the basis for the budget presentation that will enable reviewers <strong>to</strong> make judgments as <strong>to</strong><br />

the appropriateness <strong>and</strong> reasonableness of the proposed costs as they relate <strong>to</strong> the Project Description,<br />

Health Care Plan <strong>and</strong> Business Plan Each budget should be accompanied by a complete <strong>and</strong><br />

comprehensive Budget Narrative that provides an explanation for each budget line item. Applicants<br />

must present three complete 12-month budgets (For years 1, 2 <strong>and</strong> 3).<br />

The BPHC recognizes that budgeted operational costs <strong>and</strong> income may be lower in year 1 due <strong>to</strong><br />

gradual ascent <strong>to</strong> full capacity; <strong>and</strong> it is also anticipated that year 1 may appropriately include some<br />

unique one-time costs such as equipment <strong>and</strong> alterations/renovations. It is expected that the year 2<br />

<strong>and</strong> year 3 budgets, will represent the organization at full operational capacity.<br />
