Supported Discharge Service<br />
Location: Maudsley Hospital<br />
Mode: Outpatient<br />
Our service works to reduce overall length of inpatient stay<br />
<strong>and</strong> improve quality of care, by providing a care pathway<br />
back to outpatient services for young people who have been<br />
admitted as inpatients. We offer intensive, short-term,<br />
solution-focused therapeutic work including home<br />
treatment, day care <strong>and</strong> intensive case management.<br />
Interventions which may be provided<br />
• Interventions to improve young people’s access to education,<br />
housing, social care <strong>and</strong> leisure<br />
• Crisis resolution <strong>and</strong> crisis prevention<br />
• Evidence-based psychological therapies<br />
• Skills groups to develop emotional regulation capacity,<br />
mindfulness, interpersonal skills, social skills, facilitate<br />
behavioural activation <strong>and</strong> cognitive restructuring<br />
• Art psychotherapy, music therapy, occupational therapy<br />
• Improve school or vocational performance, engage with<br />
recreational activities <strong>and</strong> develop a functional support network<br />
on the basis of the family members, peers, members of the<br />
community <strong>and</strong> professionals<br />

• Adolescents (12 years), up to 18 years old<br />
• Residents of Lambeth Southwark, Lewisham <strong>and</strong> Croydon (or residents elsewhere if an assessment/consultation only is required)<br />
• Being admitted for in-patient treatment<br />

young person’s individual presentation <strong>and</strong> requirements.<br />
