in effect at the time the work related injury or illness occurred for a period of twelve<br />

(12) months. Thereafter, the employee shall be responsible to pay the full cost of Health<br />

and Dental Insurance. If such employee chooses to change his/her coverage from single<br />

to dependent or from the conventional insurance plan to the HMO or other alternative<br />

plan, the employee shall direct his/her payment of any increase in the cost of coverage<br />

to the Human Resources Director or his designee by the tenth day (10th) of each month.<br />

Failure to pay the increased cost will result in cancellation of the change in coverage.<br />

Section 13.13 High Deductible Health Plan<br />

The Employer may institute a High Deductible Health Plan, Health Savings Account<br />

and/or Health Reimbursement Arrangement in accordance with applicable laws<br />

and regulations.<br />

Section 13.14 Pharmacy<br />

Effective January 10, 2010, the cost of prescription drugs will be a co-pay of $17<br />

for generic medications or the best daily price, whichever is less, a co-pay of $17 plus<br />

20% of the cost of preferred brand name medications, and a co-pay of $17 and the difference<br />

between the cost of the generic equivalent medication and the preferred brand<br />

name medication when the physician indicates that the generic equivalent and/or the<br />

preferred brand name may be dispensed but the employee and/or dependent prefers the<br />

non-preferred brand name medication. Mail-order prescriptions will be available in<br />

ninety (90) day increments. The co-pay for mail-order prescriptions will be two times<br />

the cost of the co-pay for each category of medications as outlined above. The maximum<br />

out-of-pocket expenditures, per calendar year, over and above the co-pays for<br />

prescription drugs per employee or dependent will be Two Thousand Dollars ($2,000)<br />

with a maximum of Four Thousand Dollars ($4,000) per family.<br />

Effective January 1, 2010: $17<br />

Effective January 1, 2011: $17<br />

Effective January 1, 2012: $17<br />

Effective January 1, <strong>2013</strong>: $17<br />

Section 13.15 Vision Care<br />

The employer shall make available a voluntary vision program, 100% funded by<br />

the employee, subject to minimum participation levels required by the carrier.<br />

ARTICLE XIV<br />

WORK ASSIGNMENTS<br />

Section 14.1 Position Requirements<br />

The Employer shall maintain general position classification specifications and<br />

make them available to the Union upon written request. All employees shall be provided<br />

with a copy of their job description upon request.<br />
