PRICE PROPOSAL – ENVELOPE 1<br />
division, commission, authority, office, branch, section and political subdivision<br />
or other governmental or quasi9governmental entity;<br />
B. have, within a three9year period preceding this bid, been convicted or had a civil<br />
judgment rendered against them for commission of fraud or a criminal offense<br />
in connection with obtaining, attempting to obtain or performing a public<br />
federal, state or local contract ; violation of federal or state antitrust statutes or<br />
commission of embezzlement, theft, forgery, bribery, falsification or destruction<br />
of records, making false statements, or receiving stolen property;<br />
C. are currently indicted for or otherwise criminally or civilly charged by a<br />
governmental entity (federal, state or local) with commission of any of the<br />
offenses enumerated in paragraph (B) of this certification; and<br />
D. have, within a three9year period preceding this bid, had one or more public<br />
contracts (federal, state or local) terminated for cause or default.<br />
4. The Bidder has, or will subcontract to firms having, current, valid contractor or trade<br />
licenses and permits required under applicable New York State and New York City laws<br />
for any trade or specialty area in which the firm seeks to perform work.<br />
5. During the term of construction of the projects that comprise this package, the contractor<br />
will have in place a suitable quality control and quality assurance program and an<br />
appropriate safety and health plan.<br />
6. The amount of the Price Proposal and the value of the Bidder’s outstanding incomplete<br />
contracts does not exceed the firm’s existing aggregate rating limit.<br />
7. The undersigned understands and agrees with the terms and conditions contained within the<br />
Form of Contract and Instructions to Bidders 1, Section III, Article 6: Times for<br />
Completion and Article 7: Liquidated Damages.<br />
