projects to increase health<br />
care for the poor were<br />
on the rise. This gave all<br />
mid-level health care<br />
workers a tremendous<br />
increase in jobs and<br />
opportunities.<br />
In the early 1970s, the<br />
sense of crisis in health<br />
care was accompanied by<br />
considerable optimism<br />
about the possibilities for<br />
successful reform.<br />
In 1974, the incentives<br />
that favored hospital<br />
care promoted the<br />
neglect of ambulatory<br />
and preventive<br />
health services. The<br />
incentives that favored<br />
specialization also<br />
caused primary care to be<br />
neglected.<br />
The concept of “health<br />
care as a matter of right,<br />
not privilege” captured<br />
the spirit of the time<br />
better than any other<br />
single idea.<br />
