3. Storage & transportation<br />
Cold cha<strong>in</strong>: It is necessary to ma<strong>in</strong>ta<strong>in</strong> <strong>the</strong> cold cha<strong>in</strong> at all levels i.e. from <strong>the</strong> mo<strong>the</strong>r<br />
center to <strong>the</strong> blood storage center to <strong>the</strong> issue of blood. This can be achieved by us<strong>in</strong>g<br />
<strong>in</strong>sulated carrier boxes. Dur<strong>in</strong>g transportation, <strong>the</strong> blood should be properly packed <strong>in</strong>to<br />
cold boxes surrounded by <strong>the</strong> ice packs. Ice, if used should be clean and should not come<br />
<strong>in</strong> direct contact with <strong>the</strong> blood bags. The blood should be kept <strong>in</strong> blood bank refrigerator<br />
at 4°-6°c ± 2°c. The temperature of <strong>the</strong> blood should be monitored cont<strong>in</strong>uously.<br />
Storage: The storage center should check <strong>the</strong> condition of blood on receipt from <strong>the</strong><br />
mo<strong>the</strong>r center and also dur<strong>in</strong>g <strong>the</strong> period of storage. The responsibility of any problem<br />
aris<strong>in</strong>g from storage, cross match<strong>in</strong>g, issue and transfusion will be of <strong>the</strong> storage center.<br />
Any unit of blood show<strong>in</strong>g hemolysis, turbidity or change <strong>in</strong> colour should not be taken<br />
on stock for transfusion. Due care should be taken to ma<strong>in</strong>ta<strong>in</strong> sterility of blood by<br />
keep<strong>in</strong>g all storage areas clean. The<br />
expiry of <strong>the</strong> blood is normally 35/42 days depend<strong>in</strong>g on <strong>the</strong> type of blood bags used.<br />
The Medical Officer <strong>in</strong>-charge should ensure that unused blood bags should be returned<br />
to <strong>the</strong> mo<strong>the</strong>r center at least 10 days before <strong>the</strong> expiry of <strong>the</strong> blood and fresh blood<br />
obta<strong>in</strong>ed <strong>in</strong> its place. The blood storage centers are designed to ensure rapid and safe<br />
delivery cf whole blood <strong>in</strong> an emergency. The detail of storage of packed cells, fresh<br />
frozen plasma and platelets concentrate are <strong>the</strong>refore not given <strong>in</strong> <strong>the</strong>se guidel<strong>in</strong>es. In<br />
case, however, <strong>the</strong>se are required to be stored, <strong>the</strong> storage procedures of <strong>the</strong> mo<strong>the</strong>r<br />
blood bank should be followed.<br />
4. Issue of blood s<br />
Patients blood group<strong>in</strong>g and cross match<strong>in</strong>g should <strong>in</strong>variably be carried out before issue<br />
of blood. A proper record of this should be kept.<br />
First In and First Out (FIFO) policy, whereby blood closer to expiry date is used first,<br />
should be followed.<br />
