Statement of Work: FDIC Phase I ESA, continuedIV. Tasks, continuedvi.Polychlorinated Biphenyl (PCB)(a) Determine whether electrical or other suspectedPCB-containing equipment has been brought onto theProperty and, if so, provide the following information:i. PCB content of oils or other fluids present in theequipment (as evidenced by the presence oflabels indicating non-PCB contents);ii.iii.iv.Name of utility company that owns theelectrical or other equipment (if applicable), unitmanufacturer, serial numbers and otherdescriptive markings on the equipment;The date the equipment was manufactured; andThe location of equipment. Also identify thelocation on a real property survey and on facilitybuilding plans (on maps or schematics).(b) For all equipment on the Property identifyi. Evidence of any spills and releases associatedwith such equipment;ii.iii.Any conditions associated with the equipmentor its contents that may result in, or contributeto, future harm or damages; andThe disposal location of such equipment or itscontents if such location is on the Property.vii.Asbestos-Containing Material (ACM)The potential for Asbestos-Containing Materials(ACM) shall be evaluated for all buildings constructedprior to 1981 that have any remaining useful life,excluding single-family one-to-four units (unless thereis a state or local law requiring an asbestos inspection),and also excluding properties which have beenrenovated, and in which the asbestos has been removedand/or a current Operations and Maintenance (O&M)Program is in place.26
