Infusion Therapy: Peripherally Inserted Central Catheter (PICC) Removal _RN<br />

9. After catheter has been removed 1-2", re-grasp near exit<br />

site and repeat procedure until completely removed. If<br />

patient reports severe pain or if abnormal resistance to<br />

removal is assessed, stop procedure, secure catheter and<br />

contact physician.<br />

10. After catheter is removed, apply sterile gauze dressing and<br />

antiseptic ointment to site, and tape in place or apply<br />

transparent dressing. (Application of ointment may occlude<br />

skin tract and prevent air embolism.)<br />

11. Measure the length of the catheter, condition of tip and<br />

compare with pre-insertion length. If length is smaller than<br />

the pre-insertion length, notify physician immediately.<br />

12. Instruct patient/caregiver to assess site every 24 hours until<br />

site is epithelialized.<br />

13. Instruct patient to observe site for:<br />

a. Excessive bleeding or drainage.<br />

b. Extensive bruising.<br />

c. Pain, redness or swelling.<br />

d. Signs and symptoms of infection.<br />

e. Unusual pain or discomfort.<br />

f. Chest pain or discomfort.<br />

g. Instruct patient to leave dressing in place for 24 hours.<br />

If no bleeding or drainage occurs, site may be left open<br />

to air.<br />

14. Discard soiled supplies in appropriate containers.<br />

AFTER CARE:<br />

1. Document in the patient's record:<br />

a. Date, time and procedure performed.<br />

b. Length of catheter removed.<br />

c. Patient's response to procedure, side effects and<br />

management.<br />

d. Instructions given to patient/caregiver.<br />
