Infusion Therapy: Administration of Total Parenteral Nutrition __RN<br />

EQUIPMENT:<br />

Gloves<br />

Alcohol applicator (wipe/swab/disk/ampule)<br />

Antimicrobial applicator (wipe/swab/disk/ampule)<br />

Medications in single or multi-dose containers<br />

Syringes of appropriate sizes with 21-gauge needles or<br />

needleless adaptors<br />

TPN solution (warmed to room temperature)<br />

Puncture-proof container<br />

Impervious trash bag<br />

PROCEDURE:<br />

1. Adhere to Universal Precautions.<br />

2. <strong>Care</strong>fully read the name, dose and expiration date<br />

on each medication label.<br />

3. Inspect the TPN bag for patient name, correct<br />

formula, expiration date and leaks, and the solution<br />

for cloudiness, discoloration, sediment, particles<br />

and/or brown oily streaks (lipid solutions).<br />

4. Place the TPN bag on a clean surface with the<br />

injection port handy.<br />

5. Ensure adequate lighting.<br />

6. Wash hands. Don gloves.<br />

7. Clean injection port with alcohol applicator. Allow to<br />

air dry.<br />

8. Place the first medication container near the bag,<br />

away from the others. Check the medication label<br />

again.<br />

9. Clean top of vial with alcohol applicator. Allow to air<br />

dry. Repeat using antimicrobial applicator, if<br />

applicable. Let air-dry. DO NOT BLOT.<br />

10. Choose the appropriate-sized syringe with needle or<br />

needleless adaptor, remove the needle cover and<br />

draw the appropriate amount of air into the syringe.<br />

11. Insert the needle or needleless adaptor into the vial,<br />

below the fluid level.<br />

12. With the needle below the fluid level, withdraw the<br />

prescribed amount of medication.<br />

13. Remove the needle or needleless adaptor from the<br />

vial, insert into the TPN bag injection port, and inject<br />

the medication.<br />

14. Remove the needle or needleless adaptor from the<br />

port, and drop into the needle disposal container.<br />

15. Gently rock the TPN bag to thoroughly mix the<br />

added medication.<br />

16. Discard the used medication vial, and place the next<br />

one near the bag.<br />

17. Repeat steps 9 through 15 for each medication<br />

additive.<br />

18. Discard soiled supplies in appropriate containers.<br />

19. Remove gloves. Wash hands.<br />

3. STARTING THE TPN INFUSION<br />

Total volume to be infused: cc.<br />

Initial rate: cc/hr.<br />

Increase to cc/hr after hrs.<br />

Then cc/hr after hrs.<br />

It is common for TPN orders to contain a one hour ramp<br />

up order for the first one hour of the infusion then a one<br />

hour ramp down for the final hour. The hours in between<br />

the first and last are infused at a steady plateau rate.<br />

This way the patient is not subject to abrupt increases or<br />

drops in glucose levels from the TPN solution.<br />

EQUIPMENT:<br />

Gloves<br />

Alcohol applicator (wipe/swab/disk/ampule)<br />

Antimicrobial applicator (wipe/swab/disk/ampule)<br />

TPN solution (warmed to room temperature, with<br />

additives added)<br />

Administration set<br />

Filter, 0.22 micron for TPN, 1.2 micron if 3 in 1<br />

21-gauge needle in protective "Click-Lock" device or<br />

needleless adaptor<br />

Infusion pump on IV pole<br />

Batteries or a plug in power source<br />

Catheter clamp (optional)<br />

Tape<br />

Puncture-proof container<br />

Impervious trash bag<br />

PROCEDURE:<br />

1. Adhere to Universal Precautions.<br />

2. Place the TPN bag on a clean surface, or hang on<br />

the IV pole, as you prefer for ease of handling.<br />

3. Place patient in comfortable position, making sure<br />

that site is accessible.<br />

4. Ensure adequate lighting.<br />

5. Wash hands. Glove.<br />

6. Attach the filter and the needle or needleless<br />

adaptor to the administration set.<br />

7. Remove the cover from the port on the TPN bag,<br />

and the cover from the administration set, and insert<br />

the spike securely into the bag.<br />

8. Fill the drip chamber half way, and expel air from<br />

the tubing, filter and needle or needleless adaptor, if<br />

applicable.<br />
