presence <strong>of</strong> a policy or action plan is a yes or<br />

no; the same yes or no cannot be a response<br />

for the implementation / status <strong>of</strong> different<br />

action plans. Similar is the case for the status<br />

<strong>of</strong> implementation <strong>of</strong> legislation, or with<br />

respect to drug logistics. Thus, for such subdomains,<br />

semi-quantitative and / or ranking <strong>of</strong><br />

information has been adopted. For example,<br />

for status <strong>of</strong> the action plan, consensus is<br />

arrived regarding the components <strong>of</strong> an<br />

action plan which range from 0 to 10 and are<br />

considered as the overall situation across the<br />

state and not with respect to any one area /<br />

institution specifically. The availability <strong>of</strong><br />

drugs is categorised as always, many times,<br />

sometimes or irregular supply. The listing <strong>of</strong><br />

different registers for mental health care at<br />

different levels <strong>of</strong> care has been adopted. For<br />

many domains, all available documents were<br />

examined before accepting final information.<br />

Domain 4: This domain pertains to the<br />

management <strong>of</strong> mental health problems and<br />

is focused on documenting the burden <strong>of</strong><br />

mental health problems within the state. It<br />

collated and collected information pertaining<br />

to management <strong>of</strong> mental health problems<br />

using the ICD classification. Age and gender<br />

specific burden <strong>of</strong> suicides was also included.<br />

Supplementary and related aspects <strong>of</strong> case<br />

management like maintenance <strong>of</strong> registers,<br />

provision <strong>of</strong> drugs across the state and at<br />

different service delivery points (district,<br />

CHC, taluka, PHC, etc.,) are recorded. This<br />

section also documents information pertaining<br />

to details <strong>of</strong> camps and outreach services<br />

conducted. The key output from this section<br />

is an estimate <strong>of</strong> the total burden <strong>of</strong> mental<br />

health problems currently being served by the<br />

different health facilities within the state.<br />

Domain 5: The determinants <strong>of</strong> mental<br />

health reside outside the health sector;<br />

systems for care are incomplete if these<br />

health, and health-related sectors are not<br />

included. In addressing these concerns and<br />

issues about mental health, domain 5 collects<br />

details about the intra and inter-sectoral<br />

activities. The information from this section<br />

underscores the need for both intra- and<br />

inter-sectoral collaboration and recognizes<br />

the contribution <strong>of</strong> sectors <strong>of</strong> education,<br />

women and child development, disability /<br />

welfare and others for issues and concerns<br />

<strong>of</strong> mental health. Recognizing the difficulties<br />

in recording intra- and inter-sectoral<br />

collaboration, this domain documents the<br />

presence <strong>of</strong> activities either planned or<br />

implemented and provides explanations<br />

regarding the different sets <strong>of</strong> activities.<br />

Domain 6: This pertains to social welfare<br />

activities. Persons with mental health<br />

problems are highly vulnerable and<br />

sensitive to changes in the socio-economic<br />

sphere, thus necessitating the pro-active<br />

involvement <strong>of</strong> social assistance, assurance<br />

and other defence mechanisms to support<br />

and boost the livelihood <strong>of</strong> persons affected<br />

with mental health problems. While, there<br />

can be an array <strong>of</strong> facilities that could be<br />

provided / made available, in the current<br />

version <strong>of</strong> the SMHSA, the focus is on<br />

issuing <strong>of</strong> disability certificates, provision <strong>of</strong><br />

monthly pension / assistance, reservation in<br />

jobs and preferential allotment in housing.<br />

Domain 7: Civil society and its organisations<br />

play the role <strong>of</strong> conscience keeper. Different<br />

mental health NGOs serve to highlight and<br />

monitor, supplement the services in the<br />

government sector. Domain 7 attempts to<br />

pr<strong>of</strong>ile the different NGOs working in the<br />

state for the welfare and service <strong>of</strong> persons<br />

with mental illness<br />

Domain 8: IEC activities are an essential<br />

component <strong>of</strong> any health programme. In the<br />

context <strong>of</strong> mental health, the need for IEC<br />

activity assumes greater importance because<br />
