Parkinson’s Disease (Disorder) By: Eric Farbman, M.D. / Friends of Parkinson’s Parkinson’s disease (PD) is a chronic and progressive neurological movement disorder, meaning that symptoms will worsen over time. It is the second most common neurodegenerative disorder. Over one million people in the United States are living with Parkinson’s Disease. In most cases there does not appear to be a genetic component. There is a slight male predominance. The exact cause of PD is unknown, but it involves the death of brain cells, particularly cells in the area of the brain called the substantia nigra. These cells produce a chemical called dopamine. When enough of these brain cells are lost, the motor symptoms of PD begin to develop and include the following: 1. Tremor, mainly at rest, usually in the limbs. The tremor will often disappear when the patient uses the limb but will re-emerge upon the cessation of movement; 2. Bradykinesia – this is slowness of movement such as reduced facial expression and/or reduced arm swing when walking; 3. Limb rigidity – this is seen as stiffness in the affected side; 4. Gait/balance problems – this is often more of a problem later in the disease course. The symptoms slowly develop over years and are often different between patients. Usually the presentation is asymmetric, although there does not appear to be any pattern as to which side of the person is affected. Although there is no cure, treatment options include medications and surgery, and these can greatly improve the quality of life. While PD itself is not fatal, the disease complications can be serious. PD does not shorten life expectancy; however, it can create a decline in the quality of life. Exercise can help the quality of life in PD, and studies suggest that it may also work on the brain to slow the progression of the disease. Throughout the course of the disease there may be non-motor symptoms. Loss of smell is often one of the first symptoms. Depression is very common. The physician will assist in the treatment of both motor and non-motor symptoms. Although nobody wants to be diagnosed with anything, there are reasons to be optimistic with PD. There are many treatments currently available, and there are many research trials examining future medications and interventions.
