Pain is one of the most frequent reasons to visit health professionals. The prevalence rate of chronic pain in Germany is reported to be 17 %. The relative risk of suffering from chronic pain during a lifetime is supposed to be more than 0.80. If pain persists longer than 6 months, it tends to result in psycho-social impairment, increased disability, reduction of quality of life and increased health care costs. The International Classification of Diseases (ICD 10) classifies pain primarily in the different sections of the organic diseases as the origin of pain, i.e. low back pain or slipped disc. Often the cause of pain conditions remains unclear. There is evidence that cognitive, affective and behavioural factors play a central role in increasing and maintaining pain symptoms. Therefore many patients need additional psychiatric or psychological interventions such as cognitive–behavioural therapy. In chronic pain conditions, multidisciplinary and multimodal concepts of pain therapy have the best scientific evidence. These therapeutic approaches require a lot of medical care resources, particularly with regard to staff requirements. With the implementation of DRG systems (diagnoses related groups) as the basis of reimbursement of hospital treatment in Germany, the ICD 10 classification system became more important: What we cannot code does not exist in the reimbursement system. The bio-psycho-social dimensions of chronic pain need a multidimensional approach to the problem. Monocausal biomedical causality is as unfeasable as monocausal psychological conditions. The ICD 10 (German modification) established in 2009 the new Code F45.41 (“Chronic pain disorders with somatic and psychological factors“) that allows the coding of chronic pain conditions requiring psychologic or psychiatric intervention. In Germany, this new ICD 10 Code serves as a marker in morbidityoriented compensation systems like the diagnosis-related groups (DRG) or the morbidity-oriented risk adjustment fund.
Wil Toenders Wil Toenders Former secretary of Dutch Medicines Reimbursement Advice Committee, now independent consultant in healthcare and medicines reimbursement issues. Education: graduated as a pharmacist with a subsidiary in general practice. Work experience: after practising in a pharmacy in a healthcare centre in Amsterdam, he became editor of the Dutch Drug Bulletin in 1990 and was General Secretary of the International Society of Drug Bulletins from 1996. In 1999 he joined the Dutch Healthcare Insurance Board as a pharmaceutical adviser. From 2004 to 2009 he was secretary of the Dutch Medicines Reimbursement Advice Committee, the independent expert committee responsible for advising the Ministry of Health on the reimbursement of medicines. Present role: on 1st January 2009 he started his own independent consultancy company ToendersdeGroot, which advises on healthcare and medicines reimbursement issues. Among the activities are: participation in scientific advisory boards, advising on strategic issues, advising on the design of clinical trials, doing reimbursement feasibility studies and preparing reimbursement dossiers. 89
