4.2 OlkiluOtO NPP uNits 1 aNd 2<br />
Operation and operational events<br />
No events lead<strong>in</strong>g to a reactor trip occurred at<br />
the Olkiluoto <strong>nuclear</strong> power plant. In addition to<br />
the annual ma<strong>in</strong>tenance outages, TVO shut plant<br />
units down for repairs to a so-called ma<strong>in</strong>tenance<br />
outage on three occasions: damaged <strong>in</strong>ner parts <strong>in</strong><br />
the valves <strong>of</strong> the systems required for overpressure<br />
protection <strong>of</strong> and residual heat removal from the<br />
primary circuit were replaced at both plant units,<br />
and the motor <strong>of</strong> one reactor coolant pump was replaced<br />
at Olkiluoto 1 due to a bear<strong>in</strong>g fault. Other<br />
significant events <strong>in</strong>cluded faults <strong>in</strong> the diesel generators<br />
form<strong>in</strong>g part <strong>of</strong> the emergency power systems.<br />
The events are described <strong>in</strong> Appendix 3.<br />
<strong>STUK</strong> oversaw the operation on a daily basis<br />
at the plant site, by <strong>in</strong>spect<strong>in</strong>g the regular reports<br />
on operat<strong>in</strong>g activities as well as the event<br />
reports, and by mak<strong>in</strong>g two <strong>in</strong>spections as part<br />
<strong>of</strong> the periodic <strong>in</strong>spection programme. One <strong>of</strong> the<br />
most significant observations regard<strong>in</strong>g operation<br />
was to do with the activities <strong>of</strong> operat<strong>in</strong>g personnel<br />
dur<strong>in</strong>g the annual ma<strong>in</strong>tenance outages. There<br />
have been several events <strong>in</strong> recent years where the<br />
operators have reacted slowly to the alarms com<strong>in</strong>g<br />
to the control room dur<strong>in</strong>g annual ma<strong>in</strong>tenance<br />
outages. Similarly, there have been several events<br />
related to component isolation dur<strong>in</strong>g the annual<br />
ma<strong>in</strong>tenance outages. TVO has identified the <strong>in</strong>dividual<br />
events, analysed them and determ<strong>in</strong>ed the<br />
corrective actions. The events have, for example,<br />
resulted <strong>in</strong> the pumps <strong>in</strong> <strong>safety</strong> systems start<strong>in</strong>g<br />
unnecessarily as well as <strong>in</strong> radioactive systems<br />
be<strong>in</strong>g released <strong>in</strong>side the plant. Due to the recurr<strong>in</strong>g<br />
nature and common features <strong>of</strong> these events,<br />

entity and determ<strong>in</strong>e the corrective actions and<br />
report the results to <strong>STUK</strong> <strong>in</strong> a root cause report<br />
and dur<strong>in</strong>g an <strong>in</strong>spection to be conducted <strong>in</strong> the<br />
