establishing links with Primary care services. There is strong evidence that this does<br />

not happen often, and may contribute to high readmissions rates of Pacific children to<br />

hospital.<br />

Compared to last year, there is a noticeable trend in the increasing need for financial<br />

assistance for hospitalised patients and families. Aside from non-residents, families<br />

need help with food parcels (14) and transport. PFSU is able to access volunteer<br />

community groups e.g. International Church, Mt Eden, and local Khyber Pass St<br />

Davids Food bank.<br />

Contrary to the common view that the Pacific elderly are cared for at home by<br />

extended family members, admissions of Pacific elderly patients to ACH are showing<br />

that a greater proportion are these patients are now being discharged to rest homes<br />

or private hospitals – Family meetings and discussion by PFSU staff and Pacific<br />

families have identified that most say they cannot cope with caring for their elderly<br />

parent or family member without external assistance. In most cases, many of them<br />

are unaware of or do not know where to get appropriate support. The current HOP<br />

contracts with Pacific Provider to support and deliver Daycare services and role of<br />

the HOP Parish Community Nurse and Self Management Education Nurse are<br />

important for engaging with Pacific communities.<br />
