Los estudioshan sugerido que los efectos de laaplicación de presión negativa son:la optimización del flujo sanguíneola reducción de la inflamaciónen los tejidos de la zonala eliminación de fluidospotencialmente dañinos de la herida.( )La terapia de heridasmediante presiónnegativa estimula elcrecimiento celular.Estos cambios fisiológicos generan unentorno húmedo para la curación yfacilitan la eliminación de bacterias de laúlcera. Además, la aplicación de presiónsubatmosférica podría ayudar a aumentar elritmo de división celular y, por <strong>con</strong>siguiente,la formación de tejidos de granulación.Aunque el efecto exacto de la terapia deheridas mediante presión negativa no estáclaro, se ha sugerido que la aplicación deuna fuerza micromecánica a las heridasdeforma o estira cada una de las células,lo cual estimula el crecimiento celular y lacuración de la herida. Las ortesis de marcha limitanla propulsión al mantener el tobillo en unángulo de 90° y, de este modo, ayudan areducir la presión sobre la planta del <strong>pie</strong>.La mejor característica de las ortesis demarcha es también, paradójicamente, suposible defecto. La posibilidad de quitarseel mecanismo elimina el elemento de“obligada observancia” que es el mejoratributo de la férula de <strong>con</strong>tacto total.Férula de <strong>con</strong>tacto totalDe los numerosos mecanismos dedescarga, las férulas de <strong>con</strong>tacto totalson <strong>con</strong>sideradas por muchos como el“<strong>buen</strong> estándar” a la hora de <strong>con</strong>seguir laredistribución de la presión y la curación.El uso de una férula de escayola en eltratamiento de las lesiones neuropáticasdel <strong>pie</strong> se <strong>con</strong>oce como férula de<strong>con</strong>tacto total porque utiliza una férulabien moldeada y mínimamente acolchadaque está en <strong>con</strong>tacto <strong>con</strong> el total de lasuperficie plantar del <strong>pie</strong> y la parte inferiorde la <strong>pie</strong>rna. La estrecha adaptación dela escayola a la superficie plantar del <strong>pie</strong>aumenta el área de soporte del pesosobre la misma y ayuda a distribuir lapresión, que en vez de recaer sobreuna o dos áreas de la planta del <strong>pie</strong>se reparte por toda la superficie de lamisma. 3 Y, lo más importante, la férula de<strong>con</strong>tacto total no es de quita y pon.( )La férula de <strong>con</strong>tactototal es el “estándar deoro” en la redistribuciónde la presión.Desgraciadamente, sin embargo, tambiénhay una serie de atributos potencialmentenegativos que podrían <strong>con</strong>vencer a algunosprofesionales sanitarios para no utilizaresta modalidad. La aplicación de las férulasde <strong>con</strong>tacto total lleva tiempo y sueleir asociada a una curva de aprendizaje.Una férula que no se adapte bien puedeprovocar la irritación de la <strong>pie</strong>l y, enalgunos casos, ulceración; la mayoría delos centros médicos no cuentan <strong>con</strong> unprofesional sanitario o un técnico enférulas <strong>con</strong> la formación o la experienciaadecuadas para aplicar sin riesgos una férulade <strong>con</strong>tacto total.
