KaanSamenvattingDoel: transrectale echogeleide prostaatbiopsie (TRUS-PB)vindt doorgaans plaats onder ciprofloxacineprofylaxe.In de westerse wereld wordt een stijging gezien in hetgebruik van ciprofloxacine en de resistentie van gramnegatieveverwekkers. Wij onderzochten of er een toenameaantoonbaar is in de incidentie van infectieuze complicatiesna TRUS-PB, en of er literatuur beschikbaar is die aanpassingvan het huidige profylactische regime ondersteunt.Methoden: retrospectieve analyse van klinische en microbiologischegegevens rond TRUS-PB in het St. AntoniusZiekenhuis en literatuuronderzoek naar profylactischeregimes.Resultaten: analyse van 2736 ingrepen toonde een stijgingin de incidentie van koorts na TRUS-PB van 0,4% in 2008tot 3,0% in 2011, ondanks gebruik van ciprofloxacineprofylaxe.Ook de incidentie van urineweginfecties ensepsis nam toe, net als de incidentie van ciprofloxacineresistentieonder verwekkers van deze infecties. Er is geenliteratuur beschikbaar die de overstap naar een anderprofylactisch regime ondersteunt.Conclusie: bij patiënten met infecties na TRUS-PB iseen nauwkeurig beleid essentieel. Een urinekweek dientaltijd te worden verricht voorafgaand aan antibiotischebehandeling. Bij empirische antibioticumkeuze moetrekening gehouden worden met de kans op ciprofloxacineresistentie.Als profylaxe verdient een eenmalige doseringciprofloxacine de voorkeur. Gezien de verwachte verderestijging in ciprofloxacine-resistentie is meer onderzoeknaar alternatieve profylactische regimes noodzakelijk.TrefwoordenProstaatbiopsie, antibiotische profylaxe, ciprofloxacine,resistentieAbstractObjective: transrectal ultrasound guided prostate biopsy(TRUS-PB) is regularly performed in conjunction withciprofloxacin prophylaxis. In the western world both cipro-floxacin use and resistance among Gram-negative bacteriaare rising. We investigated whether the incidence ofinfections following TRUS-PB is increasing, and whetherevidence is available to support adjustment of the currentprophylactic regimen.Methods: retrospective analysis of clinical and microbiologicaldata of 2736 procedures conducted in the St. AntoniusHospital from 2008 to 2011, and review of literature onprophylactic regimens.Results: there has been an increase in the incidence offever after TRUS-PB from 0.4 percent in 2008 to 3.0percent in 2011, despite prophylactic ciprofloxacin use. Alsothe incidence of urinary tract infections and sepsis afterTRUS-PB have increased, as well as ciprofloxacin resistanceamong causative pathogens. No evidence is currentlyavailable to support alternative prophylactic regimens.Conclusions: in patients with infections after TRUS-PB,accurate management is crucial. Urinary cultures arealways indicated preceding antibiotic treatment. The highrisk of ciprofloxacin resistance should be taken into accountwhen starting empiric antibiotic treatment. A single dose ofciprofloxacin is the prophylactic agent of first choice.
