INGEZONDENCOMBACTEM.J.M. BontenTijdens het ECCMID <strong>2013</strong> in Berlijn presenteerde eenGriekse collega de epidemiologie van carbapenemaseproducerendeKlebsiella pneumoniae op de intensivecareafdelingvan zijn universitair ziekenhuis, een havenstadin de provincie. Van de patiënten die minimaal driedagen op de afdeling verbleven was 85% drager van eendergelijke bacterie en bij ruim een derde van hen was debacterie tevens resistent voor colistine. Gemiddeld kregendeze patiënten dagelijks drie verschillende antibioticaen de endemie bestond uit twee genotypen, ten bewijzevan falende hygiëne. Infecties met deze bacteriën warensterk geassocieerd met sterfte. De volgende dag verteldeeen Griekse intensivist op de Duitse televisie dat zijnziekenhuis, in een andere stad, geen geld meer hadvoor de meest basale medische hulpmiddelen en datmen de familie van patiënten vroeg om handschoenenvoor verpleging en artsen te kopen. Daarmee lijkt depost antibiotische regio Europa binnengedrongen en lijkende mogelijkheden voor omkering van de situatie uiterstbeperkt.Nieuwe antibioticaMet onze infectiepreventie kunnen we de storm inNederlandse ziekenhuizen nog wel een tijdje weerstaan,maar wereldwijd wordt de noodzaak van nieuwe antibioticavoor deze bacteriën steeds groter. Heel veel kandidatenhebben zich echter nog niet aangediend en de laatstefases van antibioticaontwikkeling, waarin effectiviteiten veiligheid in grote patiëntengroepen moet wordenvastgesteld, duurt vaak jaren. En ook dan kan het nogmisgaan, zoals recent met ceftobripole gebeurde. Hoewelhet fase-III-onderzoek positieve resultaten opleverde,twijfelden de FDA en EMA aan de integriteit van de dataen weigerden ze registratie.Om ontwikkeling van nieuwe antibiotica te stimuleren ende klinische evaluatie beter en sneller te laten plaatsvindenhebben de Innovative Medicine Initiative (IMI) van deEuropese Gemeenschap en de European Federation ofPharmaceutical Industry Associations (EFPIA) in 2012 hetNew Drugs 4 Bad Bugs (ND4BB)-programma gelanceerd,een privaatpublieke samenwerking van farmaceutischeindustrieën en academische partners.ND4BBOp dit moment bestaat het ND4BB-programma uit drieonderdelen. Onderdeel drie richt zich op het identificerenvan nieuwe antibiotica voor gramnegatieve bacteriën.Consortia van academische partners konden in maart<strong>2013</strong> een Expression of Interest indienen en later dit jaar zalduidelijk zijn wie het winnende consortium is. Onderdeleneen en twee zijn al eerder gestart en hadden in januari<strong>2013</strong> hun gezamenlijke startbijeenkomst.
