CHAPTER 2:<br />

Creat<strong>in</strong>g a policy environment that <strong>support</strong>s<br />

<strong>in</strong>tegrated programme strategies<br />

2.1 National policies <strong>and</strong> guidel<strong>in</strong>es on <strong>HIV</strong>/<strong>AIDS</strong> <strong>and</strong> <strong>nutrition</strong><br />

As a general pr<strong>in</strong>ciple, the policies <strong>of</strong> the host country regard<strong>in</strong>g <strong>HIV</strong>/<strong>AIDS</strong> should be followed<br />

when develop<strong>in</strong>g <strong>HIV</strong>/<strong>AIDS</strong> programmes for refugees, as the countries <strong>of</strong> asylum bear<br />

the ultimate responsibility for the protection <strong>and</strong> health <strong>of</strong> refugees they host. 12 However, as <strong>of</strong><br />

December 2003, <strong>of</strong> the 29 countries <strong>in</strong> Africa that hosted more than 10 000 refugees, only 10<br />

(34%) had national <strong>HIV</strong>/<strong>AIDS</strong> strategic plans that specifically mentioned <strong>activities</strong> for refugees.8<br />

Where national policies do not specifically mention refugees, <strong>and</strong> where refugees are<br />

systematically excluded from national <strong>in</strong>terventions, <strong>in</strong>ternationally accepted guidel<strong>in</strong>es should<br />

form the basis <strong>of</strong> programme design <strong>and</strong> implementation.<br />

Increas<strong>in</strong>gly, countries are develop<strong>in</strong>g national <strong>HIV</strong>/<strong>AIDS</strong> control policies <strong>with</strong> an explicit <strong>nutrition</strong>al<br />

care <strong>and</strong> <strong>support</strong> component. Nutritional programmes <strong>in</strong> refugee communities still<br />

tend to be based on <strong>in</strong>ternational emergency <strong>nutrition</strong> guidel<strong>in</strong>es, particularly where the population<br />

receives <strong>food</strong>, <strong>nutrition</strong> <strong>and</strong> health services from external agencies rather than the host<br />

country government. With the recognition <strong>of</strong> the close association between <strong>nutrition</strong>al status<br />

<strong>and</strong> <strong>HIV</strong>/<strong>AIDS</strong> progression, the development <strong>of</strong> national policies <strong>and</strong> guidel<strong>in</strong>es on <strong>nutrition</strong>al<br />

care <strong>and</strong> <strong>support</strong> for PLWHA is becom<strong>in</strong>g an <strong>in</strong>ternational priority.<br />

2.2 United Nations policies <strong>and</strong> guidel<strong>in</strong>es on <strong>HIV</strong>/<strong>AIDS</strong> <strong>and</strong> <strong>nutrition</strong><br />

S<strong>in</strong>ce the first UN strategy document on <strong>HIV</strong>/<strong>AIDS</strong> was published <strong>in</strong> 1986, the UN has cont<strong>in</strong>ued<br />

to update <strong>and</strong> ref<strong>in</strong>e its recommendations on how to deal <strong>with</strong> the disease. The World<br />

Health Organization (WHO) has developed an extensive literature base on <strong>HIV</strong>/<strong>AIDS</strong> care,<br />

counsell<strong>in</strong>g, test<strong>in</strong>g <strong>and</strong> treatment, as well as several key publications on <strong>nutrition</strong> programmes<br />

for emergency sett<strong>in</strong>gs. UNHCR <strong>and</strong> WFP have developed a range <strong>of</strong> operational guidel<strong>in</strong>es<br />

related to <strong>HIV</strong>/<strong>AIDS</strong>, <strong>food</strong> <strong>and</strong> <strong>nutrition</strong> <strong>in</strong> refugee sett<strong>in</strong>gs. S<strong>in</strong>ce its establishment, the Jo<strong>in</strong>t<br />

United Nations Programme on <strong>HIV</strong>/<strong>AIDS</strong> (UN<strong>AIDS</strong>) has served as a focal po<strong>in</strong>t for research<br />

<strong>and</strong> programm<strong>in</strong>g related to <strong>HIV</strong>/<strong>AIDS</strong>. A number <strong>of</strong> UN policy <strong>and</strong> programme guidel<strong>in</strong>es<br />

are available, <strong>of</strong> which the follow<strong>in</strong>g are worthy <strong>of</strong> special mention:<br />

• Food <strong>and</strong> <strong>nutrition</strong> h<strong>and</strong>book 13<br />

• Food <strong>and</strong> <strong>nutrition</strong> needs <strong>in</strong> emergencies 14<br />

• Guidel<strong>in</strong>es for <strong>HIV</strong>/<strong>AIDS</strong> <strong>in</strong>terventions <strong>in</strong> emergency sett<strong>in</strong>gs 11<br />
