Experiences from the field…<br />

Palor<strong>in</strong>ya settlement, Ug<strong>and</strong>a. At Kali cl<strong>in</strong>ic, one severely malnourished young girl was<br />

brought to the therapeutic feed<strong>in</strong>g centre by her mother, a young widow <strong>with</strong> several children<br />

at home. Discussions between the mother <strong>and</strong> the TFP nurs<strong>in</strong>g staff revealed that the death<br />

<strong>of</strong> the father from illness had left the household struggl<strong>in</strong>g to make ends meet. TFP staff reported<br />

prepar<strong>in</strong>g feeds accord<strong>in</strong>g to <strong>in</strong>ternational guidel<strong>in</strong>es to <strong>nutrition</strong>ally rehabilitate the<br />

child. However, if <strong>HIV</strong>/<strong>AIDS</strong> was an underly<strong>in</strong>g cause <strong>of</strong> mortality <strong>and</strong> mal<strong>nutrition</strong> <strong>in</strong> the<br />

family, then discuss<strong>in</strong>g <strong>HIV</strong>/<strong>AIDS</strong> <strong>with</strong> the mother, referr<strong>in</strong>g the family to a VCT counsellor<br />

(based at the same cl<strong>in</strong>ic site), <strong>and</strong> arrang<strong>in</strong>g for needed long-term care <strong>and</strong> <strong>support</strong> may have<br />

improved the health <strong>and</strong> <strong>nutrition</strong>al outcomes <strong>of</strong> family members, <strong>in</strong>clud<strong>in</strong>g the malnourished<br />

child’s sibl<strong>in</strong>gs.
