patients.<br />
To c<strong>on</strong>clude, when talk therapy is not effective in the treatment <strong>of</strong> depressi<strong>on</strong>, treatment with<br />
antidepressants is recommended.<br />
1.2.2 Introducti<strong>on</strong> to the drug methotrexate<br />
The populati<strong>on</strong> under c<strong>on</strong>siderati<strong>on</strong> in this thesis is the populati<strong>on</strong> <strong>of</strong> methotrexate users. Methotrexate<br />
(MTX) is a drug used to treat specific types <strong>of</strong> cancer, <strong>and</strong> to restrain severe psoriasis <strong>and</strong><br />
severe rheumatoid arthritis. MTX is an antimetabolite that impedes an enzyme that is vital in the<br />
creati<strong>on</strong> <strong>of</strong> DNA. Its purpose is to slow down the producti<strong>on</strong> <strong>of</strong> tissues which are growing too fast. 19<br />
In additi<strong>on</strong> to comm<strong>on</strong> side effects, such as headaches <strong>and</strong> nausea, sometimes severe side effects<br />
occur when using MTX. Am<strong>on</strong>g these are allergic reacti<strong>on</strong>s, blood in the urine, irregular heartbeat,<br />
mental changes, trouble breathing <strong>and</strong> visi<strong>on</strong> changes. It is str<strong>on</strong>gly recommended not to use MTX<br />
if the patient is or is trying to become pregnant, since MTX can cause serious damage to the<br />
foetus. 20<br />
With the menti<strong>on</strong>ed side effects <strong>and</strong> recommendati<strong>on</strong>s <strong>of</strong> treatment in mind, it is clear that MTX is<br />
a drug intended to treat serious illnesses. The effectiveness <strong>of</strong> the drugs has been documented by<br />
Yamanaka et al. (2007), who evaluate the efficacy <strong>and</strong> safety <strong>of</strong> different MTX dosages using a<br />
cross-secti<strong>on</strong>al dataset <strong>and</strong> a l<strong>on</strong>gitudinal dataset with data <strong>on</strong> Japanese patients diagnosed with<br />
rheumatoid arthritis. The cross-secti<strong>on</strong>al analysis is based <strong>on</strong> 4578 patients observed in 2003 <strong>and</strong><br />
the l<strong>on</strong>gitudinal analysis is based <strong>on</strong> 1649 patients observed from 2001 until 2005. The data are<br />
collected during biannual c<strong>on</strong>sultati<strong>on</strong>s <strong>and</strong> c<strong>on</strong>sists <strong>of</strong> the physician’s assessment, an interview<br />
with the patient <strong>and</strong> laboratory data, c<strong>on</strong>taining am<strong>on</strong>g others a blood count <strong>and</strong> urine analysis. The<br />
evaluati<strong>on</strong> is accomplished by taking advantage <strong>of</strong> the fact that the patient’s rheumatologist can be<br />
identified, <strong>and</strong> the fact that there is a difference between the dosages prescribed by the<br />
rheumatologists.<br />
By comparing the outcomes across patients <strong>and</strong> taking into c<strong>on</strong>siderati<strong>on</strong> the different prescribed<br />
dosages, the authors find that larger dosages <strong>of</strong> MTX are related to better effectiveness, but at the<br />
same time there is an increase in negative side effects. Through the self-reports it is apparent that<br />
these side effects are tolerated by the patients. It should be noted that this study is not a c<strong>on</strong>trolled<br />
trial since there is no st<strong>and</strong>ard way <strong>of</strong> treating patients with MTX that is applied by all<br />
