Background to the Report<br />

“<strong>AIDS</strong> prevention work is an issue relat<strong>in</strong>g to the quality of the population, economic<br />

development, social stability <strong>and</strong> the rise or decl<strong>in</strong>e of the country.” —Ch<strong>in</strong>ese M<strong>in</strong>ister<br />

S<strong>in</strong>ce 2002, the Freeman Chair <strong>in</strong> Ch<strong>in</strong>a Studies at the <strong>Center</strong> <strong>for</strong> <strong>Strategic</strong> <strong>and</strong><br />

International Studies (CSIS) has closely observed the <strong>HIV</strong>/<strong>AIDS</strong> response <strong>in</strong> Ch<strong>in</strong>a. We<br />

have conducted numerous research visits <strong>and</strong> organized senior-level delegations to Ch<strong>in</strong>a,<br />

while also host<strong>in</strong>g many visit<strong>in</strong>g Ch<strong>in</strong>ese officials, experts, <strong>and</strong> policymakers <strong>in</strong><br />

Wash<strong>in</strong>gton, D.C., <strong>in</strong>clud<strong>in</strong>g two Ch<strong>in</strong>ese m<strong>in</strong>isters of health. Between June 2002 <strong>and</strong><br />

December 2005, Freeman Chair staff members have made more than 25 separate trips to<br />

Ch<strong>in</strong>a, logg<strong>in</strong>g some 300,000 air miles <strong>in</strong> travel to Yunnan, Sichuan, Anhui, Hubei, <strong>and</strong><br />

X<strong>in</strong>jiang prov<strong>in</strong>ces, as well as to Beij<strong>in</strong>g <strong>and</strong> Shanghai, to underst<strong>and</strong> <strong>and</strong> assess the<br />

<strong>HIV</strong>/<strong>AIDS</strong> situation <strong>in</strong> the country. This work has resulted <strong>in</strong> two major reports <strong>and</strong><br />

numerous other monographs, research articles, congressional testimonies, policy<br />

brief<strong>in</strong>gs, <strong>and</strong> essays <strong>in</strong> both Ch<strong>in</strong>ese <strong>and</strong> English. 2<br />

With the support of generous sponsors <strong>and</strong> <strong>in</strong> cooperation with the CSIS <strong>HIV</strong>/<strong>AIDS</strong><br />

Task Force, this work has offered a rare <strong>and</strong> valued opportunity to watch <strong>and</strong> take part <strong>in</strong><br />

the remarkable expansion <strong>in</strong> the Ch<strong>in</strong>ese <strong>and</strong> <strong>in</strong>ternational response to the country’s<br />

<strong>HIV</strong>/<strong>AIDS</strong> challenge. This report reflects on those ef<strong>for</strong>ts <strong>and</strong> seeks to answer the<br />

question, “What has worked best <strong>in</strong> address<strong>in</strong>g the persistent <strong>HIV</strong>/<strong>AIDS</strong> challenges <strong>in</strong><br />

Ch<strong>in</strong>a, <strong>and</strong> what are some of the most promis<strong>in</strong>g ways <strong>for</strong>ward?”<br />

Given the vastness of the country, its rapidly evolv<strong>in</strong>g policy environment on<br />

<strong>HIV</strong>/<strong>AIDS</strong> issues, <strong>and</strong> the <strong>in</strong>creased <strong>in</strong>ternational <strong>and</strong> domestic attention focused on<br />

<strong>HIV</strong>/<strong>AIDS</strong> <strong>in</strong> Ch<strong>in</strong>a, an exhaustive <strong>and</strong> up-to-date survey <strong>and</strong> assessment of “who is<br />

do<strong>in</strong>g what where” on <strong>HIV</strong>/<strong>AIDS</strong> <strong>in</strong> Ch<strong>in</strong>a posed a difficult challenge. To narrow its<br />

scope but provide a useful survey <strong>and</strong> analysis, the report focuses on programs address<strong>in</strong>g<br />

<strong>HIV</strong>/<strong>AIDS</strong> where the disease presented the toughest immediate <strong>and</strong> long-term<br />

challenges—primarily <strong>in</strong> Yunnan, Sichuan, <strong>and</strong> X<strong>in</strong>jiang—<strong>and</strong> where the success or<br />

failure of those <strong>in</strong>terventions would ultimately shape <strong>and</strong> determ<strong>in</strong>e the future course of<br />

the <strong>HIV</strong>/<strong>AIDS</strong> epidemic <strong>in</strong> Ch<strong>in</strong>a.<br />

This research focused most heavily on prevention ef<strong>for</strong>ts rather than treatment<br />

(though recogniz<strong>in</strong>g that the presence <strong>and</strong> <strong>in</strong>tegration of both is absolutely necessary <strong>for</strong><br />

overall success). Particular attention was devoted to <strong>in</strong>ternational implementers <strong>and</strong> their<br />

partners on the premise that their experience might br<strong>in</strong>g time-tested <strong>and</strong> <strong>in</strong>novative<br />

strategies, technologies, <strong>and</strong> productive practices that would hold promise <strong>for</strong> the broader<br />

national response, especially as they are translated <strong>in</strong>to real local capacity on the ground<br />

<strong>in</strong> Ch<strong>in</strong>a. This report highlights the f<strong>in</strong>d<strong>in</strong>gs <strong>and</strong> observations from this research.<br />
