Intravenous drug use <strong>and</strong> the shar<strong>in</strong>g of <strong>in</strong>jection equipment between addicts is the<br />

most efficient means <strong>for</strong> transmitt<strong>in</strong>g <strong>HIV</strong> <strong>and</strong> has been the most common means of <strong>HIV</strong><br />

transmission <strong>in</strong> Ch<strong>in</strong>a. Although the M<strong>in</strong>istry of Public Security stated that there are close<br />

to 1 million registered drug addicts, other experts estimate that the actual number of drug<br />

users might be three to six times higher. 37 Additionally, the M<strong>in</strong>istry of Health estimates<br />

that as many as 30,000 young people become addicted to hero<strong>in</strong> every year. Although<br />

Ch<strong>in</strong>ese authorities have <strong>in</strong>vested heavily <strong>in</strong> the punishment of drug dealers, <strong>in</strong> the<br />

detention <strong>and</strong> detoxification of IDUs, <strong>and</strong> <strong>in</strong> rais<strong>in</strong>g awareness about the consequences of<br />

drug use, <strong>and</strong> <strong>in</strong> spite of demonstrated successes on a small scale, there is <strong>in</strong>adequate<br />

<strong>in</strong>vestment <strong>in</strong> drug rehabilitation services.<br />

There are scores of detoxification centers—basically prison facilities <strong>for</strong> drug<br />

addicts—operated by local public security authorities throughout Ch<strong>in</strong>a. There are also<br />

voluntary centers operated by hospitals <strong>and</strong> other governmental organizations.<br />

Involuntary detoxification centers are essentially short-term, low-security prisons where<br />

<strong>in</strong>mates are cut off from drug supplies <strong>and</strong> <strong>for</strong>ced to quit cold turkey. Detention terms<br />

vary from 3 to 12 months, <strong>and</strong> <strong>in</strong>mates are lectured about the dangers of drug use <strong>and</strong><br />

<strong>HIV</strong>/<strong>AIDS</strong>. Liv<strong>in</strong>g conditions are Spartan <strong>and</strong> detention centers vary <strong>in</strong> their capacity,<br />

depend<strong>in</strong>g on the relative wealth of the county. Some detox centers <strong>in</strong> poor counties<br />

charge <strong>in</strong>mates daily fees <strong>and</strong> f<strong>in</strong>es <strong>in</strong> order to cover operat<strong>in</strong>g costs. Recidivism rates<br />

associated with <strong>in</strong>voluntary centers are estimated to be around 90 percent, demonstrat<strong>in</strong>g<br />

that the <strong>in</strong>voluntary detox centers are, at most, successful <strong>in</strong> gett<strong>in</strong>g drug users off the<br />

street rather than off drugs.<br />

Voluntary detoxification services operate mostly <strong>in</strong> urban areas where drug addicts<br />

can af<strong>for</strong>d to check themselves <strong>in</strong> <strong>and</strong> pay daily fees. Stays are short, last<strong>in</strong>g only a week<br />

or two. Methadone <strong>and</strong> bupenorph<strong>in</strong>e are available <strong>in</strong> some centers, along with Ch<strong>in</strong>ese<br />

medic<strong>in</strong>es that help ease withdrawal symptoms, <strong>and</strong> counsel<strong>in</strong>g takes place between <strong>in</strong>house<br />

physicians <strong>and</strong> patients. These voluntary centers focus on drug detoxification<br />

rather than on long-term rehabilitation.<br />

There are very few residential drug rehabilitation centers <strong>in</strong> Ch<strong>in</strong>a, but there are<br />

exist<strong>in</strong>g, albeit expensive, models that demonstrate promise. One drug rehabilitation<br />

organization operates both street-level needle exchange <strong>and</strong> outreach as well as long-term<br />

residential rehabilitation services. For <strong>in</strong>dividuals who can af<strong>for</strong>d to pay <strong>for</strong> room <strong>and</strong><br />

board, the facility provides a drug-free home based on the therapeutic community<br />

concept, which is a highly structured family environment with considerable peer<br />

<strong>in</strong>teraction <strong>and</strong> group therapy. Residents engage <strong>in</strong> regular peer-support activities,<br />

contribute labor to the ma<strong>in</strong>tenance <strong>and</strong> operation of the facility, <strong>and</strong> are also <strong>in</strong>volved <strong>in</strong><br />

job tra<strong>in</strong><strong>in</strong>g <strong>and</strong> skills build<strong>in</strong>g to facilitate their re<strong>in</strong>tegration <strong>in</strong>to the community.<br />

Regular psychosocial counsel<strong>in</strong>g, peer counsel<strong>in</strong>g, <strong>and</strong> a positive, nonjudgmental,<br />

structured environment contribute to relative success <strong>in</strong> gett<strong>in</strong>g clients off drugs <strong>and</strong> <strong>in</strong><br />

significantly reduced recidivism rates. Comprehensive substance-abuse rehabilitation<br />

services are expensive, however, <strong>and</strong> at the moment, are limited to those drug addicts<br />
