34 <strong>Assess<strong>in</strong>g</strong> <strong>HIV</strong>/<strong>AIDS</strong> <strong>Initiatives</strong> <strong>in</strong> Ch<strong>in</strong>a<br />
NGOs rush <strong>in</strong>to new programs without accurately survey<strong>in</strong>g what programs already exist,<br />
how they operate, <strong>and</strong> what the organization can do so as to not duplicate services <strong>and</strong><br />
avoid underm<strong>in</strong><strong>in</strong>g exist<strong>in</strong>g relationships <strong>and</strong> <strong>in</strong>frastructure. Government organizations,<br />
particularly at lower levels, lack the capacity or <strong>in</strong>centive to coord<strong>in</strong>ate outside<br />
organizations, lead<strong>in</strong>g to misunderst<strong>and</strong><strong>in</strong>g <strong>and</strong> gross <strong>in</strong>efficiencies.<br />
Most importantly, <strong>for</strong> an effective, comprehensive, patient-centered approach to work<br />
with<strong>in</strong> marg<strong>in</strong>alized <strong>and</strong> at-risk populations, a far more strategic <strong>and</strong> regularized<br />
coord<strong>in</strong>ation mechanism should be <strong>in</strong>stituted at local levels where programs are be<strong>in</strong>g<br />
implemented.<br />
This can happen at two levels. First, local governments, <strong>and</strong> particularly the<br />
prov<strong>in</strong>cial, prefectural, <strong>and</strong> county-level government <strong>HIV</strong>/<strong>AIDS</strong> work<strong>in</strong>g groups, made<br />
up of representatives from all the key agencies <strong>and</strong> departments concerned with<br />
<strong>HIV</strong>/<strong>AIDS</strong>, need to be more proactive <strong>in</strong> coord<strong>in</strong>at<strong>in</strong>g their responses, both with<strong>in</strong> the<br />
government <strong>and</strong> with outside donors. Second, the donors, NGOs, <strong>and</strong> vendors, at local<br />
levels, should <strong>for</strong>m a regularized channel of discussion <strong>and</strong> coord<strong>in</strong>ation among<br />
themselves <strong>and</strong> with the local government. In some cases, one major donor <strong>in</strong> a locality<br />
might take the lead to coord<strong>in</strong>ate <strong>and</strong> contract the services of local vendors <strong>and</strong> NGOs, <strong>in</strong><br />
cooperation with local government agencies.
