6 <strong>Assess<strong>in</strong>g</strong> <strong>HIV</strong>/<strong>AIDS</strong> <strong>Initiatives</strong> <strong>in</strong> Ch<strong>in</strong>a<br />
expected to term<strong>in</strong>ate fund<strong>in</strong>g <strong>in</strong> 2006, although the UK <strong>for</strong>eign aid agency, the<br />
Department <strong>for</strong> International Development (DFID), is currently consider<strong>in</strong>g options to<br />
cont<strong>in</strong>ue fund<strong>in</strong>g programs that support <strong>and</strong> complement Global Fund–sponsored<br />
programs <strong>in</strong> seven southwestern prov<strong>in</strong>ces. In addition, it was announced <strong>in</strong> May 2005<br />
that Merck & Co., the U.S. pharmaceutical company, will work with the Ch<strong>in</strong>ese<br />
M<strong>in</strong>istry of Health to implement a five-year, $30-million program of comprehensive<br />
<strong>HIV</strong>/<strong>AIDS</strong> patient management, to <strong>in</strong>clude elements of education, awareness, prevention,<br />
health care worker tra<strong>in</strong><strong>in</strong>g, treatment, care, <strong>HIV</strong>-drug research, <strong>and</strong> job tra<strong>in</strong><strong>in</strong>g<br />
programs <strong>for</strong> recover<strong>in</strong>g drug users.<br />
This <strong>in</strong>ternational support, while deliver<strong>in</strong>g valuable fund<strong>in</strong>g <strong>and</strong> technical assistance,<br />
also provides an <strong>in</strong>creased profile to <strong>HIV</strong>/<strong>AIDS</strong> issues at local levels <strong>and</strong> pushes officials<br />
outside the health sector to take <strong>in</strong>creas<strong>in</strong>g notice of public health issues <strong>and</strong> commit<br />
greater local resources.<br />
Persistent Challenges <strong>and</strong> Gaps<br />
Despite, however, major <strong>in</strong>creases <strong>in</strong> new resources, the establishment of new programs,<br />
<strong>and</strong> an unambiguous shift <strong>in</strong> the central government’s stance toward deal<strong>in</strong>g with the<br />
disease, significant gaps <strong>in</strong> capabilities <strong>and</strong> policies exist <strong>and</strong> <strong>for</strong>midable challenges lie<br />
ahead.<br />
Poor Coord<strong>in</strong>ation with<strong>in</strong> <strong>and</strong> across Relevant Agencies <strong>and</strong> Actors<br />
Combat<strong>in</strong>g <strong>HIV</strong>/<strong>AIDS</strong> is still largely seen as a “health problem” to be tackled by the<br />
bureaucratically weak <strong>and</strong> poorly funded M<strong>in</strong>istry of Health, rather than as a broader<br />
socioeconomic challenge requir<strong>in</strong>g a more comprehensive <strong>and</strong> coord<strong>in</strong>ated response<br />
across the governmental <strong>and</strong> nongovernmental spectrum. Ch<strong>in</strong>a’s government system is<br />
highly “stovepiped,” with each government unit operat<strong>in</strong>g with<strong>in</strong> its own vertical<br />
comm<strong>and</strong> cha<strong>in</strong>, frustrat<strong>in</strong>g coord<strong>in</strong>ation between departments <strong>and</strong> across bureaus.<br />
For example, the Ch<strong>in</strong>ese government—not unlike others around the world—grapples<br />
with how best to deliver preventive <strong>in</strong>terventions to certa<strong>in</strong> at-risk groups such as IDUs.<br />
Differences persist across Ch<strong>in</strong>a between local public health <strong>and</strong> public security<br />
authorities over the appropriate response, rang<strong>in</strong>g from officially supported needle<br />
exchanges <strong>and</strong> methadone replacement therapy to “strike-hard” campaigns of<br />
<strong>in</strong>carceration <strong>and</strong> cold-turkey detoxification. Such differ<strong>in</strong>g approaches across the<br />
Ch<strong>in</strong>ese bureaucracy underm<strong>in</strong>e the government’s ability to deal with the task at h<strong>and</strong>:<br />
stemm<strong>in</strong>g the spread of <strong>HIV</strong>/<strong>AIDS</strong>.<br />
Because government-implemented programs are relatively <strong>in</strong>dependent, there is<br />
frequently <strong>in</strong>adequate oversight. Accountability with<strong>in</strong> government organizations is often<br />
difficult to assure, sometimes lead<strong>in</strong>g to <strong>in</strong>efficiencies, waste, <strong>and</strong> possibly fraud. With<br />
the new <strong>HIV</strong>/<strong>AIDS</strong> regulations issued <strong>in</strong> January 2006, greater day-to-day responsibility<br />
<strong>for</strong> implement<strong>in</strong>g prevention, treatment, <strong>and</strong> care will fall to county-level governments,<br />
which may exacerbate exist<strong>in</strong>g problems of implementation, oversight, <strong>and</strong><br />
accountability.
