24 <strong>Assess<strong>in</strong>g</strong> <strong>HIV</strong>/<strong>AIDS</strong> <strong>Initiatives</strong> <strong>in</strong> Ch<strong>in</strong>a<br />

spokesperson Pu Cunx<strong>in</strong>, highly visible <strong>in</strong> public health cl<strong>in</strong>ics <strong>and</strong> even <strong>in</strong> the departure<br />

halls of the Beij<strong>in</strong>g airport.<br />

<strong>HIV</strong> test<strong>in</strong>g is offered at many hospitals <strong>and</strong> cl<strong>in</strong>ics as well as CDC sites, but the<br />

quality, availability, <strong>and</strong> af<strong>for</strong>dability of test<strong>in</strong>g is <strong>in</strong>consistent from place to place.<br />

Despite a policy of free test<strong>in</strong>g, many venues still charge fees, <strong>and</strong> others only provide<br />

free test<strong>in</strong>g dur<strong>in</strong>g promotional campaigns. Health care workers are often unaware of the<br />

need <strong>for</strong> confidentiality <strong>in</strong> counsel<strong>in</strong>g, while most lack the tra<strong>in</strong><strong>in</strong>g to effectively convey<br />

important knowledge to at-risk persons. Local cl<strong>in</strong>ics tend to be passive about test<strong>in</strong>g,<br />

wait<strong>in</strong>g <strong>for</strong> people to come <strong>in</strong> <strong>for</strong> test<strong>in</strong>g on their own accord, rather than proactively<br />

conduct<strong>in</strong>g outreach to at-risk persons. This is partly due to the lack of political or<br />

f<strong>in</strong>ancial <strong>in</strong>centives <strong>for</strong> cl<strong>in</strong>ics to exp<strong>and</strong> free services. F<strong>in</strong>aliz<strong>in</strong>g fund allocation <strong>and</strong><br />

reimbursement policies <strong>for</strong> cl<strong>in</strong>ics will improve the application of VCT <strong>and</strong> allow <strong>for</strong> the<br />

expansion of services to a larger segment of the population. As the capacity to exp<strong>and</strong><br />

test<strong>in</strong>g occurs, however, identify<strong>in</strong>g <strong>and</strong> reach<strong>in</strong>g out to those <strong>in</strong> need of test<strong>in</strong>g will<br />

require the greater <strong>in</strong>volvement of the community, <strong>in</strong>clud<strong>in</strong>g peer- <strong>and</strong> NGO-driven<br />

outreach. But the will<strong>in</strong>gness of all government authorities to allow a greater role <strong>for</strong><br />

NGOs <strong>and</strong> other peer groups varies widely across different parts of the country.<br />

The health bureaus <strong>in</strong> many prov<strong>in</strong>ces with concentrated <strong>HIV</strong>/<strong>AIDS</strong> epidemics have<br />

actively encouraged people with <strong>HIV</strong>/<strong>AIDS</strong> to <strong>for</strong>m mutual support groups. Under the<br />

close supervision of health authorities, these groups are generally provided with meet<strong>in</strong>g<br />

space, which is used <strong>for</strong> tra<strong>in</strong><strong>in</strong>g <strong>and</strong> shar<strong>in</strong>g <strong>in</strong><strong>for</strong>mation <strong>and</strong> other social activities.<br />

Some cl<strong>in</strong>ics have made use of peer educators to reach out <strong>and</strong> identify IDUs <strong>and</strong><br />

distribute clean needles. Un<strong>for</strong>tunately, although these programs are very useful, they<br />

rema<strong>in</strong> relatively small scale—estimates vary, but there may be as many as a million or<br />

more IDUs shar<strong>in</strong>g needles <strong>in</strong> Ch<strong>in</strong>a today—<strong>and</strong> will not make a significant impact on<br />

the national scale of the spread of <strong>HIV</strong> unless such programs are dramatically exp<strong>and</strong>ed.<br />

Needle exchange is often not supported by communities or the police, <strong>and</strong> peer educators<br />

carry<strong>in</strong>g needles <strong>and</strong> narcotics have reportedly been arrested <strong>in</strong> some areas. Health<br />

authorities, however, have more strongly encouraged peer outreach activities that serve<br />

less controversial purposes.<br />

The M<strong>in</strong>istry of Health <strong>and</strong> the CDC are demonstrat<strong>in</strong>g their support <strong>for</strong> the peerdriven<br />

delivery of a variety of services. Government-controlled funds are be<strong>in</strong>g made<br />

available to NGO groups as part of each Global Fund grant. The Round 5 grant is to tra<strong>in</strong><br />

540 peer educators <strong>and</strong> 1,440 health counselors per year, over five years, <strong>in</strong> VCT services<br />

<strong>in</strong> order to reach 777,000 CSWs <strong>and</strong> economic migrants. Other funds have been made<br />

available to peer groups seek<strong>in</strong>g to reach MSM <strong>and</strong> IDU populations.<br />

This stance reflects the government’s grow<strong>in</strong>g awareness that they cannot f<strong>in</strong>d these<br />

marg<strong>in</strong>alized groups <strong>and</strong> must rely on grassroots organizations to conduct outreach <strong>and</strong><br />

provide prevention education <strong>and</strong> counsel<strong>in</strong>g. Some peer groups have demonstrated<br />

<strong>in</strong>novative outreach <strong>and</strong> VCT activities, such as one MSM group that hosts tea parties <strong>for</strong><br />

MSM <strong>in</strong> the community to dissem<strong>in</strong>ate prevention <strong>in</strong><strong>for</strong>mation, condoms, <strong>and</strong> lubricants<br />

<strong>and</strong> to collect blood samples that are numbered <strong>and</strong> sent to CDC labs <strong>for</strong> <strong>HIV</strong> test<strong>in</strong>g.<br />

The CDC provides the test results back to the peer group, which then contacts the<br />

<strong>in</strong>dividuals to <strong>in</strong><strong>for</strong>m them of the results. This process creates a welcom<strong>in</strong>g <strong>and</strong> sensitive<br />

environment <strong>and</strong> helps ensure confidentiality <strong>for</strong> the tested person.
