exchange with hidden users were arrested, reportedly because they were also carry<strong>in</strong>g<br />
drugs that the police suspect they were sell<strong>in</strong>g along with the clean needles.<br />
The net result of these stepped-up police activities will likely have a negative impact<br />
on <strong>HIV</strong>/<strong>AIDS</strong> prevention activities throughout the southwest, regardless of <strong>in</strong>creased<br />
<strong>in</strong>ternational fund<strong>in</strong>g <strong>and</strong> support <strong>for</strong> these activities.<br />
Commercial Sex Workers<br />
Aggressive suppression campaigns follow<strong>in</strong>g the Communist victory <strong>in</strong> 1949 nearly<br />
eradicated commercial sex work <strong>in</strong> ma<strong>in</strong>l<strong>and</strong> Ch<strong>in</strong>a. Commercial sex work reappeared,<br />
however, <strong>and</strong> has grown more <strong>and</strong> more evident s<strong>in</strong>ce the re<strong>for</strong>m <strong>and</strong> open<strong>in</strong>g period<br />
began <strong>in</strong> 1979. The loosen<strong>in</strong>g of <strong>for</strong>mer taboos, greater social mobility, <strong>and</strong> economic<br />
development have been factors <strong>in</strong> the rapid expansion of commercial sex <strong>in</strong> Ch<strong>in</strong>a.<br />
Today, there is a greater social <strong>and</strong> legal tolerance <strong>for</strong> commercial sex work,<br />
compared to illicit drug use or male-male sex behaviors. In many urban areas <strong>in</strong> Ch<strong>in</strong>a,<br />
commercial sex solicitation is openly conducted with many cities <strong>and</strong> even smaller<br />
county seats sprout<strong>in</strong>g <strong>in</strong><strong>for</strong>mal red-light districts. Grow<strong>in</strong>g numbers of travelers engage<br />
<strong>in</strong> sex tourism, visit<strong>in</strong>g certa<strong>in</strong> areas <strong>for</strong> the purpose of engag<strong>in</strong>g <strong>in</strong> commercial sex.<br />
Although female commercial sex work is the dom<strong>in</strong>ant <strong>for</strong>m, male commercial sex work<br />
is <strong>in</strong>creas<strong>in</strong>g, particularly among impoverished urbanites or young men from rural areas<br />
who are <strong>in</strong>duced to travel to cities to become “money boys.”<br />
There are widely vary<strong>in</strong>g estimates of the actual numbers of women who engage <strong>in</strong><br />
commercial sex work, with estimates rang<strong>in</strong>g between 3 million <strong>and</strong> 5 million. Because<br />
many women engage <strong>in</strong> commercial sex on a temporary or sporadic basis, it is extremely<br />
difficult <strong>for</strong> public health <strong>and</strong> public security authorities to determ<strong>in</strong>e either the<br />
cumulative or current number of commercial sex workers.<br />
All concerned observers agree, however, that people who engage <strong>in</strong> commercial sex<br />
work are at an <strong>in</strong>creased risk of the sexual transmission of <strong>HIV</strong> <strong>and</strong> other sexually<br />
transmitted <strong>in</strong>fections (STIs) <strong>for</strong> both biological <strong>and</strong> social reasons. In particular, many<br />
studies show <strong>in</strong>consistent <strong>and</strong> low condom use among commercial sex workers,<br />
heighten<strong>in</strong>g the risk <strong>for</strong> <strong>HIV</strong> transmission. In many cases, commercial sex workers state<br />
that it is difficult to impossible to conv<strong>in</strong>ce their sexual partners to consistently use<br />
condoms <strong>and</strong> becomes a particular problem when clients are adamant or offer more<br />
money, or when the sex worker is afraid to propose condom use to a regular partner.<br />
Additionally, some sex workers <strong>and</strong> their clients lack education on <strong>and</strong> awareness of the<br />
importance of condom use <strong>and</strong> there<strong>for</strong>e do not use them.<br />
Accord<strong>in</strong>g to the United Nations <strong>and</strong> the Ch<strong>in</strong>ese government, “approximately<br />
127,000 sex workers <strong>and</strong> their clients are liv<strong>in</strong>g with <strong>HIV</strong>/<strong>AIDS</strong>, account<strong>in</strong>g <strong>for</strong> 19.6<br />
percent of the total number of estimated <strong>HIV</strong> cases.” Government figures also show that<br />
estimated prevalence among sex workers has dramatically <strong>in</strong>creased, from 0.02 percent <strong>in</strong><br />
1996 to 0.93 percent <strong>in</strong> 2004. 26 In areas where <strong>HIV</strong> is more prevalent, particularly <strong>in</strong><br />
