Ef<strong>for</strong>ts to exp<strong>and</strong> VCT <strong>and</strong> peer education are welcome <strong>and</strong> much needed but face<br />

many obstacles. While the government officially encourages peer participation <strong>and</strong> the<br />

establishment of NGOs <strong>for</strong> <strong>HIV</strong> education <strong>and</strong> prevention work, these groups must walk<br />

a careful l<strong>in</strong>e <strong>and</strong> avoid operat<strong>in</strong>g outside of accepted (but unwritten) political<br />

boundaries. Although there is the desire <strong>for</strong> peer groups to conduct behavior studies <strong>and</strong><br />

provide counsel<strong>in</strong>g <strong>and</strong> prevention education, there is resistance to decentraliz<strong>in</strong>g the<br />

actual <strong>HIV</strong>-test<strong>in</strong>g process. Likewise, there is concern at local government levels that<br />

peer groups, <strong>in</strong>clud<strong>in</strong>g groups of people liv<strong>in</strong>g with <strong>HIV</strong>/<strong>AIDS</strong>, could attempt to exp<strong>and</strong><br />

their role <strong>and</strong> engage <strong>in</strong> advocacy <strong>and</strong> possibly criticize or challenge the government <strong>in</strong><br />

an organized fashion. In addition, the government has not yet elucidated a clear strategy<br />

<strong>for</strong> peer groups to mobilize society <strong>and</strong> <strong>in</strong>fluence <strong>and</strong> shape op<strong>in</strong>ions <strong>in</strong> the community to<br />

reduce stigma <strong>and</strong> discrim<strong>in</strong>ation. Because stigma <strong>and</strong> discrim<strong>in</strong>ation regard<strong>in</strong>g<br />

<strong>HIV</strong>/<strong>AIDS</strong> is so serious <strong>in</strong> Ch<strong>in</strong>a, it <strong>in</strong>hibits the successful implementation of many<br />

<strong>in</strong>terventions. Civil society <strong>and</strong> peer groups are well positioned to engage <strong>in</strong> stigma<br />

reduction <strong>and</strong> antidiscrim<strong>in</strong>ation <strong>and</strong> possibly <strong>HIV</strong>-test<strong>in</strong>g, but they are currently<br />

underutilized.<br />

Overall, the will<strong>in</strong>gness of some government agencies to <strong>in</strong>cubate peer groups<br />

appears driven by a need to reach <strong>in</strong>to marg<strong>in</strong>alized high-risk populations. For the<br />

successful expansion of VCT <strong>and</strong> peer education <strong>in</strong> combat<strong>in</strong>g <strong>HIV</strong>/<strong>AIDS</strong>, the role of<br />

local NGOs <strong>and</strong> peer groups, especially those associated with or made up of at-risk <strong>and</strong><br />

stigmatized populations, will need to be clarified <strong>and</strong> better <strong>in</strong>tegrated <strong>in</strong>to the broader<br />

anti-<strong>HIV</strong> ef<strong>for</strong>t.<br />

Condom Promotion<br />

Condom distribution <strong>and</strong> promotion is exp<strong>and</strong><strong>in</strong>g with condoms be<strong>in</strong>g <strong>in</strong>creas<strong>in</strong>gly<br />

available <strong>in</strong> hotels, enterta<strong>in</strong>ment establishments, <strong>and</strong> vend<strong>in</strong>g mach<strong>in</strong>es. NGOs are<br />

play<strong>in</strong>g a significant role <strong>in</strong> market<strong>in</strong>g condoms <strong>in</strong> t<strong>and</strong>em with prevention <strong>in</strong><strong>for</strong>mation,<br />

particularly among the most at-risk <strong>in</strong>dividuals who are wary of <strong>in</strong>teract<strong>in</strong>g with<br />

government authorities <strong>and</strong> representatives.<br />

The availability <strong>and</strong> widespread use of condoms is unquestionably an important<br />

component of a comprehensive <strong>HIV</strong>/<strong>AIDS</strong>-prevention campaign. Af<strong>for</strong>dability <strong>and</strong><br />

quality, widespread availability, <strong>and</strong> the awareness of the importance of condom use are<br />

critical to assure the successful control of <strong>HIV</strong>/<strong>AIDS</strong>. Condom availability <strong>and</strong> use is<br />

vitally important <strong>in</strong> areas where <strong>HIV</strong> is concentrated <strong>and</strong> where high-risk behaviors take<br />

place, such as <strong>in</strong> enterta<strong>in</strong>ment establishments. It is critical that condom use <strong>in</strong>crease<br />

among commercial sex workers <strong>and</strong> their clients, particularly <strong>in</strong> areas where <strong>in</strong>travenous<br />

drug use is common <strong>and</strong> where significant numbers of drug users earn money through<br />

commercial sex. Condom use at the nexus of these high-risk behaviors can prevent <strong>HIV</strong><br />

from bridg<strong>in</strong>g <strong>in</strong>to the general population.<br />

Ch<strong>in</strong>a has made progress promot<strong>in</strong>g the use of condoms, <strong>and</strong> <strong>in</strong> many ways, the<br />

government is well suited to promot<strong>in</strong>g condom use <strong>and</strong> is leverag<strong>in</strong>g its strength to do<br />

so. Propag<strong>and</strong>a departments are well versed <strong>in</strong> mass communication <strong>and</strong> have been<br />

successful <strong>in</strong> <strong>in</strong>creas<strong>in</strong>g <strong>HIV</strong>/<strong>AIDS</strong> awareness <strong>and</strong> condom use through high-profile<br />

appearances by politicians <strong>and</strong> celebrities, street rallies, posters, pamphlets, <strong>and</strong><br />

billboards. Government-dom<strong>in</strong>ated television has also played a role <strong>in</strong> rais<strong>in</strong>g awareness
