Critical Illness Insurance Plan<br />

A Payroll Deductible Post-tax <strong>Benefit</strong><br />

What payroll deduction premiums will I pay for<br />

this plan?<br />

You select the coverage and premium that best fits your budget and<br />

family needs. As a <strong>School</strong> <strong>District</strong> <strong>of</strong> <strong>Palm</strong> <strong>Beach</strong> <strong>County</strong> employee,<br />

your group purchasing power ensures you receive a high insurance<br />

value at an affordable cost. Speak with your Enrollment Counselor<br />

for more information.<br />

Can I continue my coverage if I terminate<br />

or retire?<br />

Yes. This plan is portable after the first payroll deduction. You can<br />

continue with the full amount <strong>of</strong> insurance coverage and arrange for<br />

premiums to be billed directly to you.<br />

Additional Riders and <strong>Benefit</strong>s<br />

Health Screening <strong>Benefit</strong> (optional)<br />

Pays the cost <strong>of</strong> one screening test per calendar year (up to $50 or<br />

$100 benefit maximum). Eligible tests include:<br />

• Low Dose Mammography<br />

• Chest X-ray<br />

• Pap Smear (women over age 18)<br />

• Breast ultrasound<br />

• Hemocult Stool Specimen<br />

• Colonoscopy<br />

• Prostate Specific Antigen<br />

• Fasting blood glucose test<br />

• Flexible Sigmoidoscopy<br />

• Blood test for triglycerides<br />

• Stress test on a bicycle or treadmill<br />

• Bone marrow testing<br />

• Serum cholesterol test to determine<br />

• <strong>The</strong>rmography levels <strong>of</strong> HDL and LDL<br />

• CA 15-3 (blood test for breast cancer)<br />

• CA 125 (blood test for ovarian cancer)<br />

• CEA (blood test for colon cancer)<br />

• Serum Protein Electrophoresis (blood test for myeloma)<br />

Waiver <strong>of</strong> Premium (optional)<br />

Premiums for base coverage and added riders and benefits may be<br />

waived if the primary insured is totally disabled as defined in the<br />

rider prior to the policy anniversary date nearest his or her 60th<br />

birthday.<br />

EZ Value Plan (optional)<br />

Inflation-fighting option that automatically increases the lump sum<br />

benefit amount <strong>of</strong> your Critical Illness policy on each <strong>of</strong> the first five<br />

policy anniversaries. <strong>The</strong> increase equals an additional lump sum<br />

amount purchased by an additional $1 per week premium for each<br />

<strong>of</strong> the first five years at the attained age <strong>of</strong> the employee when the<br />

increase goes into effect. EZ Value cannot be added after age 60.<br />

Plan Provider<br />

Trustmark Insurance Company, Lake Forest, Illinois, underwrites<br />

this plan. <strong>The</strong> A.M. Best Company, an organization that compares<br />

and rates the financial strength and performance <strong>of</strong> insurance<br />

companies, has rated Trustmark “A-” (Excellent). Policy form CACI-<br />
