Enrollment Information and Deadlines<br />

Important Dates to Remember<br />

Your Annual Enrollment dates are:<br />

October 31, 2011 through November 18, 2011<br />

Enrollment Counselor on campus<br />

October 31, 2011 through November 11, 2011<br />

(Check schedule for actual dates for your location)<br />

Your Period <strong>of</strong> Coverage dates are:<br />

January 1, 2012 through December 31, 2012<br />

Newly eligible employees must enroll online within<br />

30 calendar days <strong>of</strong> their hire or rehire date. Medical plan<br />

options are limited to electing coverage in the Low Option<br />

HMO plan for a minimum <strong>of</strong> 12 months OR Waiving<br />

medical plan coverage.<br />

New employees must also declare their tobacco status by<br />

completing a paper Tobacco Affidavit form during their<br />

New Hire Orientation session.<br />

If elections are not made within 30 days, enrollment will<br />

default to the Low Option HMO single medical plan and<br />

basic life insurance. All other coverage options will be<br />

waived.<br />

Paperless Enrollment<br />

Special NOTE: This is a paperless enrollment. You may<br />

view your 2012 enrollment confirmation online after<br />

December 1, 2011.<br />

Log onto www.palmbeachschools.org and click on<br />

the Employees tab. Click on Self Service under the Quick<br />

Link on the right <strong>of</strong> the web page. Select <strong>Benefit</strong>s/<strong>Benefit</strong><br />

Summary.<br />

Please make sure that you enter 1/1/2012 to view your<br />

2012 benefit elections. You have until December 9, 2011,<br />

to submit a written appeals directly to FBMC <strong>Benefit</strong>s<br />

Management, Inc. (FBMC).<br />

Appeals<br />

FBMC reviews and makes the final determination for all<br />

enrollment appeals based upon established guidelines.<br />

All annual enrollment appeals must be submitted, in<br />

writing, directly to FBMC no later than December 9,<br />

2011. Reported issues may not result in changes, but<br />

will be reviewed and a determination will be made. All<br />

appeal determinations made by FBMC are deemed final.<br />
