Diabetes Health Plan<br />
Diabetes Health Plan gives added benefits, helps employees save.<br />
Diabetes is a disease that affects more than 23 million Americans, and if not treated properly can lead to serious<br />
health complications. <strong>The</strong>se include heart disease, kidney disease, amputations and blindness. 1<br />
Pre-diabetes means your blood sugar is higher than normal, but not high enough for a diabetes diagnosis. You may<br />
prevent pre-diabetes from becoming diabetes by losing weight, making changes in your diet and exercising.<br />
Risk factors<br />
Many people with pre-diabetes or diabetes <strong>of</strong>ten have<br />
no warning signs and go undiagnosed for several years.<br />
You are at higher risk if you:<br />
` are overweight<br />
` are physically inactive<br />
` have high blood pressure or high cholesterol<br />
` are age 45 or older<br />
` have family history <strong>of</strong> this condition<br />
` had diabetes in pregnancy or gave birth to a child<br />
heavier than 9 pounds<br />
` are African American, American Indian or Hispanic<br />
Diabetes Health Plan<br />
<strong>The</strong> <strong>School</strong> <strong>District</strong> <strong>of</strong> <strong>Palm</strong> <strong>Beach</strong> <strong>of</strong> <strong>Palm</strong> <strong>Beach</strong><br />
<strong>County</strong> is <strong>of</strong>fering Diabetes Health Plan to our<br />
employees and their families with pre-diabetes or<br />
diabetes to help manage their conditions and reduce<br />
out-<strong>of</strong>-pocket expenses. In return for meeting required<br />
health actions, you will receive:<br />
` Reduced copayments <strong>of</strong> 50% for diabetes related visits<br />
to your doctor<br />
` Reduced copayments <strong>of</strong> 50% for select diabetesrelated<br />
medications and supplies, as well as select<br />
medications for depression, high blood pressure and<br />
high cholesterol<br />
` Wellness programs to help you manage your condition<br />
` Scorecard to help you stay on track with your<br />
health care<br />
` Reminders for essential screenings and exams<br />
Eligibility<br />
Diabetes Health Plan is available to employees and family members 18 years and older who are diagnosed with<br />
pre-diabetes or diabetes, or have fasting blood sugar in the range <strong>of</strong> either condition.<br />
