HealthyLiving-Lessons for Life Wellness Rewards<br />

Wellness Rewards will continue in 2012. In fact, Wellness Rewards<br />

has expanded to include all covered Spouses/Partners. To be eligible<br />

to receive discounted premiums in 2013 each covered employee<br />

and any covered spouse/partner will need to complete three simple<br />

steps by July 31, 2012.<br />

Take Action Early<br />

To participate in the wellness rewards program, the following<br />

CONFIDENTIAL activities must be completed by July 31, 2012.<br />

Your participation will remain confidential and reviewed only by<br />

healthcare pr<strong>of</strong>essionals. <strong>The</strong> <strong>School</strong> <strong>District</strong> will be notified only<br />

that you have completed the following steps.<br />

• Wellness Physical<br />

• Biometric Health Screenings<br />

• Online Health Assessment<br />

Wellness Physical<br />

Seeing your doctor each year to have a wellness check up make<br />

good sense. This is also a requirement to satisfy the requirements <strong>of</strong><br />

Wellness Rewards.<br />

How to Complete the Wellness Physical<br />

Schedule an appointment with an in-network doctor. You can find a<br />

in-network doctor by visiting www.myuhc.com and logging in with<br />

your UHC User Name and Password. This is not your <strong>District</strong> log in.<br />

You will then want to click on Find a Doctor .<br />

A basic annual check up is all that is required at this time. Again,<br />

preventative service should be at no co-payment.<br />

Biometric Health Screenings<br />

One <strong>of</strong> the first steps toward better health is to know your key<br />

measures. Biometric screenings include: BMI (Body Mass Index or<br />

Height/Weight), Fasting Total, HDL and LDL cholesterol, Fasting<br />

Blood Sugar/Glucose, and Blood Pressure. By knowing your key<br />

numbers you can assess your current health status and determine if<br />

you are at risk for health problems like heart disease and diabetes.<br />

Reviewing results with your doctor can provide personalized<br />

follow-up with recommended treatment and wellness tools to<br />

improve your health.<br />

How to Complete a Biometric Screening<br />

As part <strong>of</strong> an annual wellness visit at your doctor’s <strong>of</strong>fice, the doctor<br />

will measure your height/weight and blood pressure and will<br />

send you to a lab for blood work if not taken in their <strong>of</strong>fice. *Your<br />

biometric screening is part <strong>of</strong> your preventive care benefits which<br />

are covered at 100%. All Blood work must be done through a Lab<br />

Corporation <strong>of</strong> America (LabCorp) in order to count toward<br />

the Wellness Rewards program.<br />

If you do not have a primary care doctor, go to www.myuhc.com.<br />

Click on “Find Physician, Laboratory or Facility.” Select “United<br />

HealthCare Choice” for providers accepting the High Option<br />

HMO and /or Low Option HMO plans. Select “United HealthCare<br />

Options PPO” for the PPO plan.<br />

Health Assessment<br />

<strong>The</strong> Health Assessment is an online interactive tool which allows<br />

you to assess your lifestyle habits that are directly linked to<br />

health status and health care costs. <strong>The</strong>se lifestyle habits include:<br />

activity/exercise, alcohol use, back care, driving, eating, exams,<br />

self-care, smoking, stress, weight control, and well-being. <strong>The</strong><br />

results and recommendations from the health assessment provide<br />

immediate feedback to help you improve or maintain your health.<br />

It also assesses your risk for some key diseases like heart attack,<br />

diabetes, and cancer. Most importantly, your results are completely<br />

confidential and created especially for you, based on your answers<br />

to basic questions about your health habits. Once you receive the<br />

results <strong>of</strong> your assessment and set personalized wellness goals, you<br />

can begin achieving those goals. You’ll feel better about yourself<br />

and enjoy the reward <strong>of</strong> making healthy choices.<br />

How to Complete a Health Assessment<br />

To complete the Health Assessment, go to www.myuhc.com.<br />

Log-in (If not registered click “Register Now” and enter requested<br />

information to create your username and password). Click on<br />

“Health & Wellness.” Click on “Take the Health Assessment”<br />

be rewarded.<br />

Complete an online health questionnaire to help you learn your<br />

current health status and about possible health risks. Your results<br />

will be provided in an easy to use quick and private report, with<br />

tips to help you maintain or improve your health. It won’t take<br />

much more than 15 minutes to complete. (That’s about the<br />

length <strong>of</strong> four songs.)<br />
