We’ve given you a reason to smile with a selection <strong>of</strong> dental plans<br />

with exciting new enhancements. UnitedHealthcare Dental is a<br />

leading dental provider in Florida, and we are proud to have been<br />

selected as your dental carrier. We <strong>of</strong>fer four flexible dental plans,<br />

paid through a voluntary, pre-tax benefit. With all <strong>of</strong> these options,<br />

we are sure you will find a plan that meets your dental needs.<br />

You may select UnitedHealthcare dental coverage separately from<br />

your medical plan.<br />

Prenatal Dental Care Program<br />

Taking care <strong>of</strong> your teeth and gums during pregnancy is an<br />

important part <strong>of</strong> a woman’s and her unborn child’s overall good<br />

health and well-being. Experts say that disease related to the gums<br />

and tooth-support structures (periodontal disease) during pregnancy<br />

is linked to an increased risk <strong>of</strong> pre-term delivery.<br />

That’s why we’ve created a dental program which provides<br />

additional in-network preventive dental care coverage for expectant<br />

mothers. If you are in your second or third trimester <strong>of</strong> pregnancy,<br />

you are eligible for this program’s benefits as part <strong>of</strong> your benefit<br />

plan.<br />

<strong>The</strong> Four Options <strong>of</strong>fered are:<br />

Managed Care Plans<br />

Option 1 (Plan S500PB) is a pre-paid plan. This plan <strong>of</strong>fers a<br />

savings <strong>of</strong> 20% to 50% on all basic and major dental services. What<br />

you will pay the dentist on your visit is listed in your Schedule <strong>of</strong><br />

<strong>Benefit</strong>s. With this plan there are no hidden charges. Additionally,<br />

you will receive the following features:<br />

• No waiting periods<br />

• No claim forms to submit<br />

• No primary dentist selection required<br />

• Self-Referral to Specialist Dentist for a 25% discount<br />

• Defined costs on 293 procedure codes<br />

• Cosmetic procedures (teeth whitening, bonding, and veneers) are<br />

included<br />

• 25% discount on all procedure codes not listed<br />

Option 2 (Plan S700) is a pre-paid plan. This plan <strong>of</strong>fers a<br />

guaranteed savings <strong>of</strong> 25% to 50% on basic and major dental<br />

services. What you will pay the dentist on your visit is listed in your<br />

Schedule <strong>of</strong> <strong>Benefit</strong>s. With this plan there are no hidden charges.<br />

Additionally, you will receive the following features:<br />

• No waiting periods<br />

• No claim forms to submit<br />

• No primary dentist selection required<br />

On your next visit, tell your dentist that you are pregnant. Provide<br />

the stage <strong>of</strong> your pregnancy and due date, and also make sure the<br />

dentist notes your attending doctor’s or obstetrician’s name (this<br />

must be included on the claim form). All fees and expenses for<br />

cleanings, deep scaling (cleaning the teeth deeper down the tooth),<br />

debridement (removing dead or infected tissues) and periodontal<br />

maintenance will be waived, if your dentist determines you require<br />

these procedures.<br />
