decide the benefits that are right for<br />
you. <strong>The</strong> Low Option HMO or Waive<br />
Option will be the medical plan<br />
options for those newly eligible for<br />
benefits.<br />
<strong>Benefit</strong> Enhancements:<br />
• Employee premium contributions are required for all levels <strong>of</strong><br />
medical coverage<br />
• Wellness Rewards expanded to financially reward and motivate<br />
employees AND covered spouses/partners to take basic health<br />
steps to be more aware and active in their own health<br />
• Increase in Mail Order 90-day supply co-payment for all medical<br />
plans<br />
• HMO Choice 39 will be called High Option HMO<br />
• High Option HMO plan: deductible <strong>of</strong> $350 per individual and<br />
$700 per family applies to medical services outside <strong>of</strong> normal<br />
<strong>of</strong>fice visits<br />
• Deductibles apply toward inpatient and outpatient hospital<br />
services<br />
• EPO plan will be called the Low Option HMO<br />
• 401(a) contribution eliminated for Low Option HMO (formerly<br />
EPO) Single Coverage enrollment<br />
Existing part-time employees who are in an active paid position as<br />
<strong>of</strong> Dec. 31, 2011, will be grandfathered in and remain eligible for<br />
benefits.<br />
• New eligibility definition – New (non-CTA) employees must<br />
work 30 or more hours per week to qualify for insurance.<br />
• Minimum $100 per month increase for part-time employees<br />
enrolled in the High Option HMO plan (lower contribution<br />
available in the Low Option HMO)<br />
• NEW Nurse at Work Program – read more about this new<br />
program on page 58.<br />
• Rates for the Dental PPO plans will increase<br />
• EyeMed benefits slightly enhanced<br />
• MetLife Long Term Care (LTC) – no new policies sold after<br />
8/1/2011. Existing policyholders can expect increased rates.<br />
