Voluntary Universal Life Insurance Plan<br />
What about the group term life policy<br />
I already have with the <strong>School</strong> <strong>District</strong>?<br />
This Universal Life Insurance plan complements any group term life<br />
insurance you may have, and enables you to vary your premiums,<br />
coverage, and cash value accumulation as your needs change. You<br />
can adjust the death benefit and premium upward and downward<br />
throughout your lifetime, subject to certificate limits.<br />
What if I become disabled?<br />
You can have your premiums waived in case <strong>of</strong> total disability with<br />
the optional Waiver <strong>of</strong> Premium rider, available to ages 18-60.<br />
Your Counselor can discuss with you the features and costs for this<br />
additional rider.<br />
What payroll deduction premiums will<br />
I pay?<br />
You select the coverage and premium that best fits your budget and<br />
family needs. As a <strong>School</strong> <strong>District</strong> <strong>of</strong> <strong>Palm</strong> <strong>Beach</strong> <strong>County</strong> employee,<br />
your group purchasing power ensures you receive a high insurance<br />
value at an affordable cost. Speak with your Enrollment Counselor<br />
for more information.<br />
Remember, this contract is <strong>of</strong>fered in addition to any employer<br />
coverage and is paid solely by the employee through post-tax<br />
contributions.<br />
How do I make changes to my coverage?<br />

Plan Provider<br />
Trustmark Insurance Company, Lake Forest, Illinois, underwrites<br />
this plan. <strong>The</strong> A.M. Best Company, an organization that compares<br />
and rates the financial strength and performance <strong>of</strong> insurance<br />
companies, rates Trustmark “A-” (Excellent).<br />
Universal Life Insurance is available on a post-tax basis, and<br />
a separate application is required. This employer-provided<br />
information is in advance <strong>of</strong> more complete coverage information<br />
from the insurer.<br />

Employee 18 to 80<br />
Spouses 18 to 70<br />
Children 0 to 23*<br />
*Children and grandchildren are eligible the day after they leave the hospital.<br />
