Extensive Personal Care: This need refers to people who do not ambulate, require<br />

total assistance and care, and/or receive enteral (tube) feeding. Seventy-three (73)<br />

residents <strong>of</strong> <strong>Lanterman</strong> (19%) require extensive personal care as <strong>the</strong>ir primary service<br />

need.<br />

Significant Behavioral Support: This need addresses individuals who have<br />

challenging behaviors that may require intervention <strong>for</strong> <strong>the</strong> safety <strong>of</strong> <strong>the</strong>mselves or<br />

o<strong>the</strong>rs. Ninety-one (91) residents (23%) have been identified as requiring significant<br />

behavioral support as <strong>the</strong>ir primary service need.<br />

Protection and Safety: This refers to those individuals who need a highly structured<br />

setting because <strong>of</strong> a lack <strong>of</strong> safety awareness, a pattern <strong>of</strong> self-abuse or o<strong>the</strong>r behavior<br />

requiring constant supervision and ongoing intervention to prevent self-injury. One<br />

hundred twenty-five (125) <strong>of</strong> <strong>the</strong> residents (32%) require highly structured services as<br />

<strong>the</strong>ir primary service need.<br />

Low Structured Setting: This service need addresses those consumers who do not<br />

require significant behavioral support or intervention but do require careful supervision.<br />

Only four residents at <strong>Lanterman</strong> (1%) are in this category.<br />

PLANNING FOR RESIDENT RELOCATION PERSON BY PERSON<br />

Stakeholder input has been significant regarding <strong>the</strong> closure plan and more specifically<br />

as it relates to <strong>the</strong> men and women who live at <strong>Lanterman</strong>. The vast majority <strong>of</strong> input<br />

has come from families <strong>of</strong> <strong>Lanterman</strong> residents and facility employees. Overall, input<br />

received has raised concern and /or opposition to <strong>the</strong> closure. However, many have<br />

recommended that, should closure be approved, a number <strong>of</strong> issues should be<br />

addressed to ensure a safe and successful transition <strong>for</strong> residents. Based upon <strong>the</strong><br />

lessons learned from <strong>the</strong> Agnews closure, and <strong>the</strong> recommendations shared by those<br />

providing input to <strong>the</strong> Department on this proposal, <strong>the</strong> following priorities have been<br />

included as primary foundations <strong>for</strong> this plan:<br />

• Decisions will be based on individualized planning that ensures a safe transition <strong>for</strong><br />

each individual. <strong>Closure</strong> will not occur until appropriate services, as identified in <strong>the</strong><br />

individual plan, are available ei<strong>the</strong>r in <strong>the</strong> community or at ano<strong>the</strong>r developmental<br />

center and <strong>the</strong> resident has moved.<br />

• Community resources, including residential and day services, will be developed.<br />

• The necessary health and medical services will be arranged within <strong>the</strong> local<br />

communities.<br />

• Behavioral and crisis support services must be available.<br />

• Ongoing oversight and monitoring must occur to ensure that <strong>the</strong> quality <strong>of</strong> care and<br />

services continues to meet <strong>the</strong> needs <strong>of</strong> persons served after transition.<br />
